KNOW ALL MEN BY THESE PRESENTS:

For value received, in the amount of Twenty Thousand Pesos

(P20,000.00), receipt of which in full is acknowledged, I, Joanna D.
Macabebe, of legal age, and residing at Juniper St., Purok Carnation, Brgy.
Bulak, Bacolod City, Negros Occidental, hereby forever remised, released
and discharged, and by these presents do, for myself, my representatives,
agents heirs, executors, and administrators, remise, release and forever
discharge ACE Corporation, its successors and assigns, and/or its directors,
officers, stockholders and employees, of and from any manner of action or
actions, cause or causes of actions, sums of money, accounts, damages,
claims and demands whatsoever, in law or equity, which, against said ACE
Corporation, its successors and assigns, and/or its directors, officers,
stockholders and employees, I ever had, now have, or hereafter may have,
or which my representative, agents, heirs, executors and administrators can,
shall or may have, upon and by reason or upon any matter, cause or thing
whatsoever in connection with ACE Corporation.

I further warrant and represent that I will not institute and hereby
forever waive any action, whether legal, administrative or of any other
nature and kind whatsoever which had, now have or hereafter may have
against ACE Corporation, its stockholders, officers, directors, agents or
employees arising out of, in connection with, related, incidental or as a
result of my assignment thereat.

Finally, I declare herein that my receipt of the aforesaid amount was

voluntary as well as the Waiver of all claims which I might have against
ACE Corporation, and that nobody forced me to execute this document, the
content of which were fully explained to me in the dialect that I understand
before I signed the same.
