AFFIDAVIT OF UNDERTAKING

I, JAY MARK C. HILARIO, of Legal Age, Filipino and a resident at Block

2008 Ipili St. Sta Cruz Manila City, after having been duly sworn to in accordance
with the law, depose and say:

1. That I am the Registered Owner of KJM Enterprise at 701-B. Evangelista St.

Quiapo Manila City, a business enterprise duly registered to engaged in the
selling of Power tools and equipment;

2. That the KJM Enterprise does not use Collection receipt for
acknowledgement on receipt of cash and/or payment of goods sold;

3. That the KJM Enterprise uses Sales Invoice and not Collection receipt;

4. That the Sales invoice is attached in this affidavit;

5. That this affidavit is being executed to attest to the truth of the foregoing facts
for purposes in relation to any concern regarding BIR transactions.
