AFFIDAVIT OF QUITCLAIM AND RELEASE

I, AGUSTIN P. REYES, of legal age, Filipino, and resident of

Catbalogan City, after having been sworn according to law, hereby
depose and state, THAT –

1. I have been an employee of D’ PROVIDER ENTERPRISES

at Catbalogan City since 2000 and that I am about to retire this year;

2. I acknowledge that I have been paid my wages/salaries and

other benefits satisfactorily by my employer for the past several years
that I have been employed with him;

3. I further acknowledge to my full satisfaction the amount of

ONE HUNDRED THOUSAND PESOS (Php 100,000.00) in
consideration of my retirement from my employment with D’ Provider
Enterprises;

4. In view hereof, I hereby release and discharge my employer

D’ Provider Enterprises or any of its officers from any and all money
claims or otherwise due me incident to my previous employment with
it;

5. I hereby further state that I have no more claim or right of

action of whatever nature whether past, present or contingent against
D’ PROVIDER ENTERPRISES.
