MEMORANDUM OF AGREEMENT

DECLARE AND MANIFEST:

1. That the PARTY OF THE FIRST PART, in his/her capacity as the President of Kalinga State
University enters into an agreement with the PARTY OF THE SECOND PART and PARTY OF
THE THIRD PART

2. That the PARTY OF THE SECOND PART is the Student petitioner requesting for the offering
of the unprogrammed subject(s) specifically

1. That the PARTY OF THE FIRST PART, thru the Vice-President of Academics and Student
Development has approved the offering of the unprogrammed subject(s)

2. That the PARTY OF THE SECOND PART (Student Petitioner) agrees to pay all financial
obligations as specified and stipulated in Article 4 Section 2:2:3 of Student Handbook
Revised Edition 2012, in the amount of

3. That the PARTY OF THE SECOND PART (Student Petitioner) must attend and complete the
required total number of lecture hours or laboratory contacts hours, pass all examinations
and accomplish satisfactory all requirements as prescribed in the subject course. However, if
he/she fails to attend/complete required total number of lecture or laboratory contact hours
or fails in the subjects, he/she shall pay fully all his/her financial obligations.

4. That the PARTY OF THE THIRD PART (Subject Teacher) shall complete the required
teaching hours and shall be paid Php. 400.00 per contract hour and that every absence will
be deducted correspondingly as reflected in the accomplished Daily Time Record duly signed
by the Dean and submitted to the Office of the Director for Finance and Administration.

5. That the PARTY OF THE THIRD PART shall hold classes with the student-petitioner(s)
outside the regular working hours. However, if he/she schedules classes within the regular
working hours, it shall be considered as tantamount to double pay hence he/she will not be
paid.

6. That the PARTY OF THE FIRST PART, in his/her capacity as President of College should
revoke or invalidate this Memorandum of Agreement in case the PARTY OF THE SECOND
PART of PARTY OF THE THIRD PART violates or fails to comply with the terms and
agreements stipulated herein.
