Logrolling
Purpose: A technique used to turn a client a client whose body must at all times be kept in straight alignment (like a log), i.e., back surgery or spinal injury. Considerable care
must be taken to prevent additional injury.
Preliminaries
Assemble all the necessary supplies and equipment.
Perform hand hygiene and observe other appropriate infection
prevention procedures.
Provide for client privacy by drawing the curtains around the bed or
closing the door to the room.
Rationale: This is a personal matter.
Demonstrate courtesy, introduce yourself and verify the client’s
identity. Explain to the client what you are going to do, why it is
necessary, and how the client can cooperate or participate.
Rationale: To establish rapport and allay fear and anxiety.
Performance
- Adjust the head of the bed to a flat position or as low as the client can
tolerate.
Rationale: This provides a position of comfort for the client.
- Raise the bed to a height that will align to the waist- or hip-level or elbows
of the caregivers.
Rationale: Ensure personnel safety.
- Be sure the wheels of the bed are locked and raise the rail on the side of the
bed opposite you (if one-care method).
Rationale: To prevent the bed from moving against the direction of the
movement.
- At least three nurses are needed to logroll the client. One nurse will stand at
the head part of the client and act as the leader or in-charge of the team.
Ensure that there is a pillow between the legs. The nurse will hold the head
of the client to maintain stability. Two nurses will stand at the side of the
bed and support the far shoulder, hip, and knee and cross over their arms .
- The nurse in-charge counts: “one, two, three, go.” Then at the same time,
all staff nurses pull the client to the side of the bed to a lateral position by
shifting their weight to the back foot.
Rationale: The position of the hands supports the client at the heaviest
parts of the body, providing greater control in movement during the roll.
Moving the client in unison maintains the client’s body alignment.
