Logrolling
Purpose: A technique used to turn a client a client whose body must at all times be kept in straight alignment (like a log), i.e., back surgery or spinal injury. Considerable care
must be taken to prevent additional injury.

2. Client’s ability to understand instructions and willingness to participate.
3. Client’s degree of comfort or discomfort when moving. If needed,
administer analgesics or perform other pain relief measures prior to the
move.

- Encumbrances to movement such as an IV or an indwelling urinary a client
catheter.
- Medications the client is receiving, because certain medications may
hamper movement or alertness of the client.
- Assistance required from other personnel. At least 2-3 additional people are
needed to perform this skill safely

Rationale: Moving a client is not a one-person task. During any client

handling, if the caregiver is required to lift more than 35 lb of a client’s
weight, then the client should be considered to be fully dependent and
assistive devices should be used. This reduces risk of injury to caregiver.

Preliminaries
 Assemble all the necessary supplies and equipment.
 Perform hand hygiene and observe other appropriate infection
prevention procedures.
 Provide for client privacy by drawing the curtains around the bed or
closing the door to the room.
Rationale: This is a personal matter.
 Demonstrate courtesy, introduce yourself and verify the client’s
identity. Explain to the client what you are going to do, why it is
necessary, and how the client can cooperate or participate.
Rationale: To establish rapport and allay fear and anxiety.

Performance

 Position yourself and client appropriately before performing the

move. Other individuals stand on the opposite side of the bed.

- Adjust the head of the bed to a flat position or as low as the client can
tolerate.
Rationale: This provides a position of comfort for the client.

- Raise the bed to a height that will align to the waist- or hip-level or elbows
of the caregivers.
Rationale: Ensure personnel safety.

- Be sure the wheels of the bed are locked and raise the rail on the side of the
bed opposite you (if one-care method).
Rationale: To prevent the bed from moving against the direction of the
movement.

- Place the client’s arms across the chest.

Rationale: This ensures that the arms will not be injured or become trapped
under the body when the body is turned.

- At least three nurses are needed to logroll the client. One nurse will stand at
the head part of the client and act as the leader or in-charge of the team.
Ensure that there is a pillow between the legs. The nurse will hold the head
of the client to maintain stability. Two nurses will stand at the side of the
bed and support the far shoulder, hip, and knee and cross over their arms .
- The nurse in-charge counts: “one, two, three, go.” Then at the same time,
all staff nurses pull the client to the side of the bed to a lateral position by
shifting their weight to the back foot.
Rationale: The position of the hands supports the client at the heaviest
parts of the body, providing greater control in movement during the roll.
Moving the client in unison maintains the client’s body alignment.

- Smoothen the linen.

Rationale: To prevent wrinkles and prevent pressure injury.

- .Place supportive devices (pillows) at the back of client’s head, back,

upper, and lower extremities.
Rationale: To maintain proper alignment.
- Raise the side rails.

Rationale: To prevent falls.

 Ensure client comfort and linens free of wrinkles.
