TRAINING HOLDING PERIOD AGREEMENT

Date: Feb. 10, 2022

I, Jason C. De Dios (Name), Project Nurse (Position)

EHS / Alta Spatial (Dept/Project) hereby agrees to abide by the stipulated

conditions of this Training Holding Period Agreement.

Terms & Conditions

A. Holding Period
In consideration of the payment by Dreambuilders Pro Inc. (DPI) for the cost of training, the
following schedule will apply:

Training Title Cost of Training Holding Period

BOSH training (OHNAP) Below 5,000.00 - 6 months
_______________________ P5,001.00 - P10,000.00 - 12 months
____________________ P10,001.00 - P15,000.00 - 16 months
____________________ P15,001.00 and above - 24 months
The concerned employee shall continue working with the Company until the end of the Holding
Period from date of finish of the training.

B. Termination of Employment
In case of voluntary or involuntary termination of employment within the holding period,
concerned employee shall reimburse DPI of the cost of the training or on a pro-rata basis
corresponding to the remaining portion of the holding period. The reimbursement will be
deducted from his/her last pay and if there is any deficiency, concerned employee shall pay
within 5 days without need of any demand.
