This questionnaire is designed to assist the Project SPLIT identify specify tasks that
Contract of Service staff needs to perform better to successfully meet or exceed their
performance goals or targets. The data that will be collected will be used as a reference in
identifying gaps in competencies and appropriate learning interventions that will respond
to the learning needs of current and future project staff. All responses/information
provided will be treated in strictest confidence and will not in any way be used to assess the
performance on the job of the respondent.
