GENERAL NOTE

To complete the proposal, the Bidder must fill-up the annexed data sheets as per the instruction given
below and also in various section of this Tender Document.

Tenderer shall furnish the data required above by typing in appropriate places against each item in the
proforma. These must be properly signed by an authorized representative of the Tenderer as token of
verification of the data submitted. These signed pages in their entirety shall be returned with and shall
form a part of Tenderer's original proposal. Failure to comply with above requirements may result in
rejection of the Tender.
