1. INTRODUCTION

Veteran Affairs Nebraska-Western Iowa Healthcare System (VANWIHS) has a

requirement to purchase JSN: M4831 – Drill, Surgical, Electric Drive, Handheld from an
authorized vendor to supply and deliver the item(s) identified in Table 1 below.
This requirement is in support of a Stryker brand name or equal requirement. See
salient characteristics.

This requirement is in support of the activation of VA Nebraska-Western Iowa Healthcare

System Ambulatory Care Center. The new Ambulatory Care Center has the need for Drill,
Surgical, Electric Drive, Handhelds for the Ambulatory Surgery department. The current
manufacturer is the basis for design for newly constructed space.

1.1 Additional Item Requirements

For all equipment, the vendor shall provide user manuals that describe proper use, care
and cleaning for users of the equipment.

In addition, the vendor will provide service manuals that includes theory of operation,
maintenance requirements, repair information, schematics, part lists and troubleshooting
information in electronic format for use by the VA Biomedical Engineering Department.

Date of manufacture must not be greater than 2 years before the purchase order date.
The equipment must not be used or refurbished and must have the latest manufacturer’s
software/hardware updates installed. Any batteries provided with the unit will not be
manufactured more than 12 months before the purchase order issue date.

The vendor shall provide, at no charge, at least 3 complete and unabridged sets of
operator manuals, service manuals, electronic schematics, troubleshooting guides and
parts lists for each piece of equipment purchased for the medical center. These manuals
will include all components and subassemblies, including those not manufactured by the
vendor. These manuals and documentation shall be identical to the ones supplied to the
manufacturer’s service representatives and shall contain the diagnostic codes, commands,
and passwords utilized in maintenance, repair and calibration of the equipment. The
vendor shall disclose at the time of quoting of this equipment any post warranty charges
such as remote service or application access charges, telephone technical or application
support charges, or any fees associated with supporting this equipment including the
hourly rate for onsite maintenance service, and the cost and description of the various
maintenance coverage products.

2. GENERAL CONDITIONS
2.1 Site Address
The address of the VA Nebraska-Western Iowa Healthcare System Ambulatory Care
Clinic is provided below.

2.2 Delivery Schedule

Items shall be physically delivered no earlier than the estimated delivery start date and no
later than the delivery end date listed in Table 2 below. The site requires initiation of the
project management support of this contract immediately in order to coordinate the

construction engineering required to support the VA construction management team
guiding the site contractor.

2.3 Schedule and Order Management

2.3.1 The above delivery schedule is based upon the current construction schedule,
which is subject to change. The vendor shall not increase prices for a change in delivery
date for up to 30+ days due to construction delays.

2.3.2 The awardees actual delivery dates will be confirmed by VA upon award.

2.3.3 Post-award, the vendor will coordinate delivery identified in this document with the
VANWIHS prior to beginning any work (See Section 2.4). Once the dates have been
confirmed, modifications to the schedule are subject to written approval by the CO, COR
and the vendor. Schedule changes that are beyond the dates specified in the contract will
require approval by the Contracting Officer and a contract modification.

2.4 Delivery and Receiving

2.4.1 Delivery and receipt of the proposed items is anticipated to be directed to the
location identified below. Confirmation of delivery location will be provided post-award. To

paragraph 2.1.1)

2.4.2 The vendor is required to define the need for a staging area to accommodate item
delivery, if needed. Post-award, this information will be confirmed with the vendor.

2.4.3 The delivery of items identified in this document shall take place during normal
business hours which are defined as: 8:15 AM to 3:00 PM Central Standard Time*,
Monday through Friday, and excluding Federal Holidays.

*COVID-19 NOTE: Due to COVID-19, the delivery business hours may change.
Currently, the adjusted delivery hours are 1:00 PM – 5:00 PM Central Standard Time. In

 addition, delivery personnel may be required to wear masks and follow other COVID-19
safety precautions that are in place at the time of delivery, to include screening if entering
the VA facility. Please confirm delivery date and time and any COVID-19 related safety
requirements in place with Omaha POCs identified in 2.4.1 prior to delivery. The vendor is
responsible for ensuring its employees and/or subcontractors have the required personal
protective equipment such as masks when on-site at the VA facility.

2.4.4 Delivery trucks will not be permitted to remain at the loading dock. Trucks shall be
unloaded, moved from the dock, and then brought back to the dock if required to haul out
any waste, tools, or excess materials.

2.4.5 Labeling of delivered items shall include the VA facilities contract number and VA
purchase order number for identification and reference upon receipt of product.

2.5 Use of Warehouse

If the vendor requires the use of the Government’s activation contractor’s warehouse to
meet the requirements of this contract the vendor must provide a certificate of insurance
prior to the delivery and offloading of the item(s). This insurance certificate must be
completed and presented to the activation contractor prior to delivery.

The vendor shall communicate through the VA and IOT&A POC any required coordination
requirements.

2.6 Clean Up and Disposal

2.6.1 There are no dumpsters available for vendor use.

2.6.2 The removal of waste and/or excess material shall be conducted through the loading
dock area.
