1. GENERAL
PT. Wijaya Karya (Persero), Tbk and MAN Industries (hereinafter WIKA-MAN) is
constructing a Gas Turbine power plant in Sumbawa, Bima and Maumere,
Indonesia. WIKA-MAN Scope is EPCC (Engineering – Procurement –
Construction and Commisioning). The scope of the duties and the
responsibilities of both parties are in detail defined in the Agreement

1.1 Legislation and codes

Indonesian laws, regulations and codes are ruling in the project.


The contents of this Site SHE Plan shall not overrule valid local legislation or
codes, unless it is agreed in the contracts between WIKA-MAN and Suppliers or
Contractors. In the event that codes other than Indonesian being adopted, the
works still shall conform to any codes and standards applicable in Indonesia or
regulations necessary for permitting and approvals. The contents of this Site
SHE Plan shall not be weaker than any valid local legislation or codes

1.2 Scope of SHE Plan

This Safety Plan is shall aply for WIKA-MAN, Contractor and subcontractors.
Contractors means the Company who has entered into the Contract with
WIKA-MAN for the partial execution of the P4 Power Plant Project.

Subcontractor means the Company who has entered into the Contract with
the Contractor who having the Contract with WIKA-MAN for partial
execution of the P4 Power Plant Project.

The Contractors are obliged to fulfil all the general and specific safety and
environmental instructions and instructions given in the Site Instructions.
The Contractors shall require their Subcontractors to follow these same
instructions. The Contractors has full responsibility for the management of
health and safety on site,
and shall co-ordinate the activities of all sub-contractors to ensure that the
requirements of the contract are satisfied, compliance with all aspects of

Health & Safety legislation and authoritative guidance, and the establishment
and maintenance at all times of a safe and healthy environment.

When a Contractor for the particular works has been chosen, a Contract’s
Kick-Off meeting shall take place. As a standard procedure in Kick-Off
meeting, also the SHE issues will be discussed and clarified. The above
mentioned documents shall be reviewed, thoroughly discussed and
compared with the procedures in use of the Contractor. This is in order
to make sure that the Contactor has understood the Employer’s
requirements.

1.3 The purpose of the SHE Plan and SHE objectives

The purpose of the SHE Plan is to assist in achievement of WIKA-MAN’s


Sustainability Policy and associated Envi ronmental and Safety Guiding
Principles which are

 Loss time injury = 0


 Frequency rate (FR) < 8 per million work hours
 incidents/ fires & leaks 0
 licence non-compliances 0
 number of safety walks ≥ 1 per week/ site managers, discipline
managers, site supervisors safety officers and foremen
 number of near misses and improvement ideas

Site SHE Plan gives to all parties at site the general guidelines and instructions
in the environmental, health and safety issues and it concerns all operations at
site.

Every Contractor and Subcontractor is responsible for maintaining good SHE


practice at site in general and particularly at its own working areas. Every
Contractor shall nominate a responsible Site Safety Officer to take part in SHE
actions and to coordinate the safety activities of his own and subcontractor’s
activities and joint site safety walks.

The guidelines and instructions in Site SHE Plan and the local SHE
regulations in Indonesia shall be followed during the implementation of the

project. Site Instructions will be issued separately and Safety Induction shall
be arranged for all contractors working at site prior to starting the works.

2. ORGANIZATION CHART

2.1 General Organization Chart


P4 Power Plant Project will lead by Hadi Irawan as Project Manager and oversees all
function. SHE function should be independent that stand on their consideration to reach
zero accident. The contractor and subcontractor under WIKA-MAN Management should
have Competent Safety Personel.

2.1.4. Project Manager

The Project Manager is responsible for the overall project and as such also for
complying with the requirements of the project specific SHE-Documents. The
implementation of SHE requirements is monitored based on an internal MAN project-
specific SHE Directory.

Responsible for :

1. Enforce rules and principles of SHE according Indonesia Regulation and Project
rules.
2. To be a role model for every person in P4 project
3. Be a main responsible person in term of OHS / SHE
4. Supprting SHE Section in purpose reach a goal (Zero Accident).
5. Implementing and ensuring compliance with the SHE-Documents in the project.
6. Variations from the rules and requirements of the project-specific SHE-Documents
must be approved by the Project Manager. These approvals must be provided in
writing.

2.2.2. Site Manager

The Site Manager is responsible for carrying out all activities on the construction site and for the
implementation of the requirements of the project-specific SHE-Documents in accordance with
the progress on site. Regular inspections must be made with the SHE Manager.

Responsible for :

1. Introduction and implementation of the project-specific SHE-Documents


2. Co-operation with the SHE Manager and Coordinator.
3. Incorporation of SHE matters int
4. o the regular site meetings with the consortium partners/companies/subcontractors.
5. Providing the necessary resources (e.g. training, emergency facilities, First-Aider training,
company doctor, etc.) and space as well as taking measures for fulfilling and complying
with the SHE requirements (also with respect to subcontractors).
6. Participant in SHE program as required (meeting, patrol, trainng, etc)

2.2.3. Leader of all Other Section

The site supervisors are responsible for monitoring the construction, installation and
commissioning activities on site. They are therefore responsible for implementing the
requirements of the SHE-Documents in their own areas. They are supported in this by the SHE
Coordinator on site.

Responsible for :

1. Ensure all employee and worker comply the project rules.


2. Ensure, work can started safety.
3. Ensure all personel worn PPE (Personel Protective Equipmment) as required
4. Pasticipant in SHE program as required.
5. Ensuring orderliness and cleanliness in their areas of responsibility.
6. Take preventive action to eliminate unsafe act and unafe condition.

2.1.4 Site personnel and all persons employed or present on

site
As a part of project team, all personel should comply the project rules an pass the induction
process first, before start to work. Anyone who take the violation shall get the penalty, until
blocked from job site.

2.2 SHE Organization Chart


SHE organization lead by SHE manager who supervise and control all SHE Policy and
Program at P4 power plant project. Each site project will lead by SHE Coordinator who
have responsible for program and policy impllementation. SHE Coordinator will assisted
by Safety Supervisor, who concern for compliance for safety aspect at the work area.
Safety Inspector, who concern to ensure all equipment and work area in safe condition.
Environment Officer, who concern for environmental aspect and keep the work area
accordance as principles of 5R. Safetyman and enviroman shall assist the.

2.2.1. SHE Manager

The SHE Manager is responsible for compile, coordinating and documenting all necessary SHE
activities in conjunction with the site.

Responsible for :

1. Complile the program, activity, schedule and resources in term SHE aspect.
2. Ensuring the provision of project-specific SHE-Documents
3. Monitoring aplication of all SHE program.
4. Compile documents related SHE.
5. Together with Project manager and site manager, compile project rules, according
Indonesia law regulation.

2.2.2. SHE Coordinator

The SHE Coordinator is responsible for implementing, coordinating and documenting all
necessary SHE activities in conjunction with the site.

Responsible for :

1. Aplly the SHE Program.


2. Take execution for any violation and devation.
3. Ensuring all project rules is adhered by all project personel.
4. Supporting SHE Manager in term of OHS aspect.
5. Compile all document related SHE.
6. Support in SHE Manager to conduct necessary action in purpose incrasing SHE performance.

6.1. SIT (Safety implementation Team)

SIT is a committe in a project that foccuses on Safety, Health and Environment aspect.
SIT consist of several function (Procurement, Commercial, QA-QC, Construction /
Production, Bussiness & Finance). So expected to create Safety, Conducive and
Productive atmosphere.

3. GENERAL RULES
General Rules for all personal at the P4 power plant project :
1. Every employee and worker who will entering the job site at first time should follow safety
induction process.
2. Every kind of work should have approved permit to work.
3. Harmfull material, narcotics, alcoholic liquor, weapon, forbbiden entering the job site.*)
4. Compulsive to follow Safety Morning Talk and Toolbox Meeting.
5. Smoking only in smoking area. **)
6. Obligate maintain the cleanliness working area.
7. Beside electrical personel, forbiden to turn on, turn off and change any electrical and
machinery parts.
8. Obey all safety direction & instruction
9. Required wear Personel Protective Equipment according kind and work location.
10. Obey all safety sign and direction on working area.
11. Work as your authority only. ***)
12. Use equipment, machine and tools as designation.
13. Keep company’s goods and inventory in good condition.
14. Do not take any action than can harming yourself and other people.
15. All criminals action will be processed by police.

*) Prohibition of usage of drugs and alcohol


Any usage of drugs, alcohol and weapon on site is strictly prohibited.
Usage of these substances will result an immediate dismissal from the
site.

Smoking is allowed only at the particularly marked smoking areas.

Hereunder are listed the main common works with particular
competence requirements requiring also a licence:

Before commencing any of these works the Contractor shall ensure that the
workers have the required and valid licences. Inspection of the valid licences shall
be carried out by the Contractor’s Site Management. Copy of the licences shall
be issued to WIKA-MAN Site Management for information and checked up. Any
equipment connected to the licence shall be certified for periodical testing. Copy
of the test protocol shall be issued to WIKA-MAN Site Management for
inspection and checking up.

4. SHE PROGRAM
8.1. Safety Induction1

Is an introduction of policies and regulation regarding Safety, Health and Environment


to all employee, and guest whot first time they entering the project area.

4.2 Safety Morning Talk and Toolbox Meeting2


Safety Morning Talk (SMT) is a sharing forum before start the activity. SMT is conduct 3
times in a week and should follow all personel unders WIKA-MAN.

Toolbox meeting (TBM) is a briefing each group about Safety, Health Environment &
Quality and progress before start the activity. TBM is conduct everyday and should
follow all personel unders WIKA-MAN, Toolbox meeting lead by Supervisor.

4.3 Safety Manajemen Patrol


Safety management patrol follow by all management level of WIKA-MAN conduct every
week for review latest condition of job site and for sweeping unsafe action, unsafe
condition and any other deviation on the job site.

4.4 General Housekeeping


Is a activity when doing every week, conduct by all worker, staff and management
together clean up the job site. Management level shall be role model for 5R (Ringkas,
rapi, resik, rawat, rajin) aspect. General Housekeeping lead by Bussiness Personnel.

4.5 Daily Safety Walk Supervision & RCA3


WIKA-MAN ’s and contractor’s Site Managers, Site Supervisors and foremen shall carry out
daily safety supervision in order to detect and prevent accident in advance and to
enhance work safety. SWSR lead by SHE Coordinator.

4.6 SHE Meeting


Conduct every week by all safety and production personel. to follow up all safety
finding, safety program, safety-production progress and schedule. SHE Meeting lead by
SHE Coordinator

4.7 SHE Inspection


Inspecting activity about work situation (by RCA instrument), Equipment (heavy
machine, fire extinguisher, electrical equipment, etc) and environment (physic
environment condition). Inspection finding shall discuss in the safety meeting.

Inspection conduct by each related function (equipment, tools, machine by mechanic,


environment by environment officer, etc). For inspection indicator will using colour code
system.

RCA is a instrument for safety checking for latest condition. All safety finding will discuss
in safety meeting.

4.8 Safety Training


For special kind of work, need equip with special skill and knowledge. For avoid any
accident happen, some training shall follow by all related worker. The training lead by
Competent person kind of work that need to safety training before start to work are :

3. Scafolding install-uninstallation
4. Confined space
5. Driving
6. Heavy equipment operation

4.9 Simulation of Emergency Situation and Fire Drill


For preparing all people in the job site, if suddenly have a emergency situation. WIKA-
MAN consider to conduct the simulation and fire drill at least 3 time in a year. The
simulation start with announcement, training and practice. Kind of emergency drill

3. Riot / caos incident

Emergency response team will be selected according safety consideration. Emergency


response plan and emergency response team will described in chapter.9.

5. STANDARD PPE (Personal Protective Equipment)


As last step of hazard controlling WIKA-MAN established standard PPE for all personel who
entering the work area.

Each Contractor shall provide for own employees the following mandatory
personal protective equipment as a minimum:
 Personal identification card
 Safety helmet
 Safety footwear
 Overall uniform
 Hearing protection whenever the noise is equal to or exceeds 85 dB
 Eye protection when working arround high pressure,
 Full body harness when working at height

Additional protective equipment shall be provided as the circumstances may demand,

e.g. respirator, safety harness and lifelines including fall arrestors and masks for welding.
Safety equipment shall meet the requirements of Indonesia.

5.1 Safety Helmet4


Is a basic PPE, everyone who will go to the work area should wear safety helmet. Safety
helmet equip by elastic chainstrap.

For Identification and Clasification purpose each contractor will have different helmet
colour.

5.5 Full Body Harness8


Mandatory PPE for work at height. Fulll body harness should have two lanyard, common
safety belt forbidden to use in this project. Lifeline and / or anchor point must installed
and complement each other to avoid worker fall from height.

Full body harness must be worn at 2 meters height or more, near hole and edge of
working platform.

6. PERMIT TO WORK (PTW)9


PTW is a instrument system that aims to give a safety guidelines for all worker and employee. So
they know what the requirement that must be completed before start working.
PTW should be made every week and when there is have a new kind of work, such as:

7. STORAGE AND TRANSPORT OF GOODS AND MATERIAL


All contractors of P4 Power Plant Project is responsible for the proper storage of his
materials and tools in the place indicated by the Site Facility Layout. The Contractors must
mark their warehouses and special working premises. Special attention must be paid to
storing of dangerous materials and to the fire prevention.

All supplies (material, equipment, and tools, etc.) transported to the site must be marked
clearly and visibly and be accompanied by appropriate documents.
The recipient as well as the contents of the shipments must be clearly recognisable. The
recipient has to ensure that transport, handling, provision or storage of the shipment is
carried out orderly and correctly.

The recipient must report to AF Site Management the goods that are considered as being
hazardous prior to bringing onto the site. Handling of any hazardous substances must
comply with the control of substances hazardous to health regulations, the
classification, packaging & labelling of dangerous substances regulations and the road
traffic (carriage of dangerous substances in packages, etc.) regulations.

6.1 Hauling away from site

Contractors may haul materials, equipment, tools, etc. away from the site only under
controlled circumstances.
For taking out materials, the Contractor’s responsible person shall prepare a list of
materials with clear identification. A written permission approved by the AF
representative is required. This permission with the list of materials shall be handed
over to the Site Security for inspection when the materials are taken out.

8. OCCUPATIONAL SAFETY
8.1 Accident prevention
Within his scope of supplies and services, the Contractor is responsible for
carrying out all necessary steps for accident prevention and labour protection in
accordance with the legal stipulations and contractual conditions.

To prevent accidents, appropriate facilities and arrangements shall be


provided and actions taken in accordance with the health & safety at work and
other relevant health
& safety legislation applicable to this type
of work.

time call upon the Contractors to perform any activity deemed necessary. In case
the Contractor does not take the necessary actions within the specified time,
any representative of the WIKA-MAN Site Management will have authority to
stop the work if, in his opinion, it is necessary for safety reasons.

All Contractors shall provide the first aid facilities on the site in accordance
with the local laws and regulations. They should also provide within their own
workers sufficient number of persons, who are trained and are having the first
aid skills.

In the event of accidents or at a general danger and distress, everyone has the
duty to provide help to the extent that is necessary and can in the
circumstances be expected from him in particular without neither running
any considerable personal risk nor neglecting other important duties.

The Contractor himself must carry out all records and legal notices of the
accidents. Calling the authorities in accordance with the local regulations
shall be of each Contractor’s duty.

8.3 Reporting and investigation of accidents, incidents and near-


misses
All accidents, incidents (fires, uncontrolled leaks > 500 l) and near-misses
must be reported immediately to WIKA-MAN Project Manager, Site
Manager, WIKA-MAN SHE Cooordinator and the Contractor’s Site Manager.
All accidents, injuries and near-misses shall be investigated in order to find
out how similar incidents could be avoided in the future. WIKA-MAN SHE
Coordinator together with the Contractor’s responsible person(s) shall initiate
and conduct the investigation and keep WIKA-MAN’s Project Manager informed
of the progress and final results. All serious accidents and incidents shall be
investigated by WIKA-MAN in accordance with WIKA-MAN investigation
procedure. Additionally WIKA-MAN Management may consider and decide
that this procedure shall be followed in investigation of any other single
accident or incident.

8.4 Workplaces and facilities

Unauthorised persons are not permitted to enter the erection areas, storage
areas and restrooms of others or to step on the construction scaffolding or
erection scaffolding of other Contractors. The representatives of the
Contractor, WIKA-MAN personnel, guarding personnel and the authorities have
always access to the working places and facilities of the Contractors, in order
to observe that if the safety regulations are followed.
The Contractors are responsible for ensuring that the entire area of their
construction and erection sites will be safeguarded in such a way that there is no
danger even if, the staff is temporarily absent.
This must particularly be observed in the case of construction and erection
work performed above pits, which are open or covered by gratings or the like,
and also on covers that, owing to their structure, do not provide sufficient
protection against the falling down of any such parts as bolts, tools, welding
electrodes, etc.

8.5 Machines and equipment

No lifting equipment, excavators, trucks and trailers or other mobile machinery


shall be used on site unless a test certificate in accordance with statutory
requirements is available. The Contractor is responsible for ensuring that test
certificates are maintained and the prescribed register is duly completed during
the contract period and renewed as required by current legislation to maintain
validity.

The user shall guarantee that the equipment has been tested, inspected, and
maintained according to the relevant regulations. The user of equipment bears
the responsibility for the sufficient carrying capacity, suitability, safety, and
proper handling of equipment and all the necessary protective facilities.

If it is necessary to attach pulley blocks, rope pulleys or similar equipment


to any structural parts, approval from the Contractor’s Site Management and
from the WIKA-MAN Site Management must in all cases be obtained before
work is started. This also applies to the attachment of stay ropes for erection
and lifting masts to be connected with building structures.

All tools, tackles and hand machines shall be in proper condition and shall only
be used for their intended purposes.

The Contractors shall continuously monitor the condition of used tools,


tackles, and machinery on the site and defective items shall be withdrawn
from the usage.

8.6 Scaffolding works, working at height

Qualified scaffolding workers in accordance with statutory requirements shall


only be entitled to erect scaffoldings and guard scaffolding on the site.

Scaffolding will be inspected and certified at weekly intervals by a competent


engineer of the Contractor and the accredited representative of the scaffold
company.

The scaffolding shall be inspected before taken into use and labelled with
inspection card. Should any fault be found in the inspections, the fault must
be corrected, before any personnel may work on the scaffolding.

Each person who uses the scaffolding shall be responsible for its proper
use. When working at above 2 metres height, rails shall be used to protect
workers from falling. If this is not possible and the worker shall work closer
than 1,5 metres to the edge, safety harness shall be used.

In all cases when working above 2 metres height a person shall ware a safety
harness and the harness shall be connected with a lifeline to a safe connection
point.

8.7 Lifting works

Before starting the lifting works, the Contractor shall present a plan for lifting
works to WIKA-MAN Site Management. The lifting works shall be planned so
that there is no risk for existing constructions, equipment or people. The
bearing capacity of the floor or the ground shall be assured. The operation
of the machinery (e.g. breaks, safety device) shall be checked daily by the
operator of the machine.

8.8 Electrical safety & illumination

8.8.1 Electrical safety during the construction works

Rules and regulations of electric safety established by authorities must be


followed. The specialists with sufficient qualification must carry out
electric works on the site during construction works.
Unauthorised persons are not admitted to enter electric rooms of the
site. If the object of works is located in the electric rooms, the schedule
and the safety measures must be agreed with an electric safety expert
(a representative of the Employer or Operator).

8.8.2 Temporary cabling, electric tools and equipment on


site

The site distribution boards and temporary cabling shall be


arranged in an orderly way and no cabling shall be on ground
without a proper protection. Temporary cabling must be done
thoroughly to avoid danger. The way of cable installation must
prevent their mechanical wearing.

All electric tools must be protected by isolation.

The Contractors guarantee that all used electric equipment and the
all mechanisms with electric drive are approved according to the
standards in force. The most dangerous in operation are the hand
electric tools with electric drive with the voltage 230 V or 400
V. When working with them the protective isolation must be
used as a measure of protection.

The usage of a damaged or a faulty electric tool, equipment, and


cables is forbidden. The Contractor shall take care that the
damages are rectified or otherwise the faulty equipment must be
immediately removed from the site.

Control panels, projectors, electric heaters, blowers, and other


equipment used on the site must be located in such a way that
allows preventing danger of inflammation or accident.

8.8.3 Connection to the power supply on site

Electrical power main distribution board for the construction


purposes will be arranged by the Contractor. The Contractors shall
arrange their own sub-distribution boards and connection lines to
those from the main distribution board.

The power requirements and power connections must be


registered with WIKA-MAN Site Manager. The electrician
employed by the Contractor shall perform the connection to

distributions belonging to the site power supply.

8.8.4 Illumination of work places

The illumination of workplaces is the duty of the Contractors.


The illumination and electrical tools used must meet the statutory
regulations or other equivalent regulations.

8.8.5 Site general illumination

Site general illumination will be organized by WIKA-MAN

8.9 Work in confined spaces

Work shall be handled as a work in a confined space if it

 contains or has potential to contain a hazardous atmosphere


 contains material that has potential to engulf the person
 has an internal configuration where the person may get trapped or
suffocated

Confined spaces include boilers, tanks, pipes, channels, pits etc. The work
shall be carried out with a written work permit defining the safety issues to be
followed. Rescue arrangements shall be carefully planned in advance and a
standby person shall be in place. It must be ensured that the conditions in the
confined space are safe throughout the work. The workers shall be equipped
with a portable oxygen measuring device and in case of any risk of flammable
liquids and/or gases, a portable LEL measuring device. Protective activities
according to statutory regulations, local safety codes or other equivalent
regulations must be taken in the case of work with lighting devices and
electric tools in confined spaces with conductive structural parts.

10. FIRE SAFETY


Fire Fighter device should provide by all contractor who have fire risk. Such as
 Fire extinguisher
 Fire blanket
 Water canon / water pump
The fire fighter device must maintained well and inspect every period.

The Contractors are responsible for fire protection on their areas (working, erection,
store, etc.) according to legal stipulations and contractual agreements. The responsible
Contractor shall take care of fire-fighting measures and shall provide the necessary
equipment, materials, fire extinguishers and first aid supplies within his area. This
equipment must be located in the especially assigned and easily available places.
Where such works that require special arrangements cannot be avoided, the agreement
with the WIKA-MAN Site Management must be obtained in advance.

10.1 Preventive measures

The preventive measures given hereunder shall be followed as a


minimum:

 Only the minimum amount of flammable material is kept at working place

 Flammable material is not kept or stored in places where it is difficult to


see it or below open stages and floors or near buildings.

 Where reasonable or practical, flammable material or products are


replaced with non-flammable material or products.
 Flammable material must be disposed of as soon as
possible.
 Stores, offices or workshops must not be established or maintained in welding
areas.
 Special danger points shall be identified and reported to the Site
Managemen.

10.2 Fire protection equipment

The fire extinguishing devices shall be kept in operational condition and


shall be a subject to weekly inspections. They shall not be covered by other
items, removed or damaged and the signs identifying the location shall not be
covered.
In case a fighting facility is damaged, the damage must immediately be
reported to the Contractor’s Site Management and WIKA-MAN Site
Management. Discharged fire extinguishers shall be replaced without delay.

10.3 Emergency exits and other routes


All emergency exit routes, walkway routes, traffic and transport routes shall
be kept clear for undisturbed passage. No obstructions causing a danger or
undisturbed passage shall be placed on these routes.

10.4 Actions in case of fire

Each worker must immediately undertake all necessary steps in case of


discovering fire, threat of inflammation or other danger.
These steps are:
 Save people who are in the immediate danger
 Close the doors and windows to prevent fire and smoke to spread further
 Warn other people and leave the place using staircase, not elevator
 Contact Site Security immediately and do the fire alarm
 Warn other people who are not in immediate danger

 Extinguish the fire and limit its spreading if you consider it to be possible, but be
careful

A manually activated fire/evacuation alarm system will be installed at site. On


hearing the fire/evacuation alarm, all personnel must leave buildings and
proceed to the nearest Assembly Point. The location(s) of the alarm and
Assembly Point(s) are indicated in the Site Facility Layout.

Each fire must be reported immediately to the Contractor’s Site Management


and WIKA-MAN Site Management, with precise indications of the place, the

object, and the extent of the fire.

The WIKA-MAN Site Management will give further instructions after due
consultation to the Contractor’s Site Management.

When fire is suppressed, it shall be guarded by the responsible


contractor, unless otherwise agreed, for at least 8 hours or more.

10.5 Fire protection inspections

Each Contractor must ensure the routine inspection of the devices. In


weekly site inspections, following items shall be checked:
 order and tidiness
 flammable liquids and combustible material
 exit routes
 portable fire extinguishers
 signs and labels
 fire doors and fire sections
 hot work places

10.6 Training

Each contractor is responsible for training of it’s own personnel.


Site supervision and workers shall be familiarised in usage of portable fire
extinguishers and general follow up of possible fire hazards. All personnel shall
be informed about the location of the telephones as well as about the numbers
to dial in a case of fire.

10.7 Hot work permits, planning and implementation of hot works

Hot work has to be done in accordance with the valid statutory requirements and
other applicable stipulations. All hot works require a Hot Work Permit.

At welding and gas-cutting works, special care must be maintained. In case when these

works (for example, welding, mechanical grinding, and other works that can cause
inflammation) are carried out inside a room from which combustible materials cannot
be removed and where exists a danger of fast fire spreading, fire-protection
measures must be agreed with the WIKA-MAN site management.

Mobile fuel gas supply facilities must be set up at the workplace or near to
it. They should if possible be in the employee’s field of vision and be easily
accessible. They should not be set up in cramped areas or other dangerous
workplaces.
LPG and acetylene fuel gas supply facilities must be safeguarded against
flashback. In the case of electric welding, strict attention must be paid to
ensure that the earth lead
may only be connected to the object to be welded at a point near the weld. It
must never be connected to any other component.

When electric arc welding or oxygen-arc cutting is performed in the rooms or


areas where the surrounding can conduct electricity, a proper grounding of
the equipment shall be maintained. Conditions are cramped and mobility
limited, DC welding generator sets with no-load voltages not exceeding 100
V may only be used. AC welding generator sets are only permissible when their
no-load voltage, for frequencies up to 50 Hz, does not exceed 42 V (42 V marking).
Welding rectifiers may only be used.

if they bear the appropriate identification mark. The performer of the work must
safely exclude the possibility of danger to third persons, to the plant or to
other facilities resulting from flying sparks, welding spatter arc flash, etc.

10.8 Handling and storage of flammable substances

The Contractors must provide to WIKA-MAN Site Management an inventory


of hazardous substances used on site, together with assessments for the method
of use in accordance with the regulations.

Gas vessels (cylinders) shall be stored in the storage in stable vertical


position. The cylinders of different type of gases should be stored separately
as well as the empty cylinders must be stored separately from the filled ones.

The cylinders being in use on site shall be installed in vertical position and
secured from falling; they must be set on the special carriages or other devicece
ensuring stable position of cylinder. The cylinder lever must be in the valve. In
connection with the danger of valve inflammation a fire- resistant glove is
attached to each cylinder.

When the works are finished the cylinder valves shall be closed. At the end of
working day the hoses shall be winded on the carriage, and not left hanging on
the floor or on other struct.

11. INCIDENT MEASURES


Every incident must discuss at the meeting to get the lesson learn and all related function must
take the follow up action so that incident not happen in next day.
Investigation process and report must be complied before 24 hours after incident.

The following incidents shall be reported immediately to WIKA-MAN Site Manager,SHE


Coordinator and to WIKA-MAN Project Manager further to report to WIKA-MAN
Management:
 Injuries leading to at least one day absence from work
 Fires
 All the leaks of oil or other chemicals to the environment

 SHE report is a part of the weekly report and cumulative events will be
collected currently to the statistics.

14. ENVIRONMENTAL GUIDELINES


Security and safety has close link. Security have ensure that only authorized people who
can entry the job site, esspecially high risk area. to contro l that, WIKA-MAN

14.1 General site rules

Clean site is an important part of good environmental practice, and also helps
everyone to maintain an efficient and safe site. The Contractor shall
 Clear waste frequently from site.
 Keep the site tidy and clean.
 Manage the material and plant storage areas properly.
 Keep the vandals away by securing the site, valuable materials and plant.

14.2 Collecting and disposing of waste

Each Contractor is responsible for collecting and sorting his own waste.
Organic, inorganic and hazardous wastes shall be sorted and delivered to the
designated disposal areas.

The Contractors are required to clean and clear their working areas daily and
remove all debris. If it is found necessary, WIKA-MAN Management will
organise collective cleaning and waste removal days. The costs of the above-
mentioned will be solely in Contractors responsibility.

14.3 Managing materials

To reduce waste amounts:


 The receiver shall be prepared for deliveries.
 Materials shall be handled properly to reduce damage or spillage.
 Damaging materials should be avoided when unloading deliveries.
 Suppliers’ instructions on material storage shall be followed.
 Valuable materials shall be stored in a secure area.

 Materials shall be stored away from waste storage containers and


vehicle movements which could cause accidental damage.

 Lightweight materials shall be secured to protect them from loss, wind


damage or causing pollution.
 Special care shall be taken when storing materials that could cause
pollution.

14.4 Managing oils, fuels and chemicals

The Contractor shall take measures to prevent accidents, injuries and spills
from oils, fuels and chemicals that are being stored and used on site:
 Oils, fuels and chemicals shall be stored away from watercourses and
drains, and in a location where they will not get knocked over.
 Large quantities shall be stored in tanks that are protected from damage.
 Refuelling shall be always supervised.
 Drip trays shall be used underneath mobile plant like generator.

 All chemicals shall have identification labels on containers. If other


containers are used those shall also be furnished with a similar label.

 The manufacturer’s storage instructions, instructions concerning


safety and handling of waste shall be followed.

 The contractors shall carry out risks assessments, implement proper


countermeasures and make appropriate safety equipment available to the
workers.
 People involved in chemical handling shall get information about the risks
and right working methods.

 During the construction phase the contractors are responsible for


handling of chemicals they use and shall have adequate identification,
risk assessment and material safety data sheets available. The
contractors are obliged to provide this information to WIKA-Man Site
SHE Officer for registering.

 AF Site Management team shall establish and maintain a chemical


register at the site where the name of the chemical and the name of the
corresponding contractor
are listed. Additionally WIKA-MAN Site Management team shall file the
material safety data sheets accordingly.

14.5 Water and watercourses

Pollution shall not enter any watercourse. Waste water shall be disposed of
properly from site. The Contractor shall take measures to avoid:
 Spillages of pollutants directly into watercourses, drains or onto paved
surfaces.
 Poor storage of oils, fuels and chemicals.
 Damage to drums, tanks and pipes, either as a result of an accident or
vandalism.
 Uncontrolled surface water runoff from site roads, excavations or disturbed
ground.
 Pumping dirty water into watercourses or surface drains.
 Working in, over or next to watercourses.

14.6 Managing silty water

The Civil Constructor must dispose of silty waters properly. The amount of
silty water shall be minimised by:
 Keeping the site roads and hard standings clean from mud.
 Stopping water entering excavations.
 Controlling surface water runoff from earthworks.

 Providing pump sumps in excavations.

In case of disposing of silty water, considerations shall be made not to


cause further pollution at the pumping area.
14.7 Emergency response in case of a spillage

Instructions for the person who is the first on the scene of a spillage:
 Remember safety first – use personal protective equipment.
 Identify the source of the pollution and stop it if possible.
 If you are unsure whether hazardous chemicals are involved call
your own Site Manager or WIKA-MAN Site Manager.
 Ensure that Site Management is told immediately.
 Avoid the spillage spreading – use sand or earth to dam the flow.
 Switch off sources of ignition.
 Do not wash the spillage into the drainage system.
 Never use detergents to clean the affected area.
 Shovel contaminated sand or earth into sacks or skips. These must be
disposed of appropriately.

14.8 Noise and vibration

The Contractors shall in the execution of the works avoid, as much as


possible, nuisance caused by noise or vibrations to the outside
communities. In case there is a particular reason for executing the works
outside the normal working hours, it shall be separately agreed with the
WIKA-MAN Site Management. In case there will be disturbances to the
outside communities, the Contractor is responsible for agreeing of the
needed actions.

14.9 Dust and emissions

The Contractors shall take care of preventing the dust spreading to the
neighbouring areas. The suitable wetting of the construction ground areas
shall be maintained by the Contractors.

The Contractors shall take care of preventing the emissions from e.g.
vehicles, mobile cranes and such similar equipment using diesel fuels.
Engines shall not be kept running unnecessarily.

14.10 Ground contamination

The Contractors shall take care of preventing any contamination of the


ground on site, and at the vicinity of the site.
The Contractors are responsible for cleaning any contaminated areas.

15. SECURITY AND ACCESS CONTROL


WIKA-MAN security guards will be maintaining the Site Security Management System
(SMS). SMS is a part of management that includes planning, implementation,
maintenance, assessment to realize a safe, safe and productive work environment. This
system shall be in operation 24 h/day during the entire construction period. Security
personel should have Certificate of Competence (Garda Pratama / above) at least Chief
& Group leader

Security and safety has close link. Security have ensure that only authorized people who
can entry the job site, esspecially high risk area. to control that, WIKA-MAN
management use some instrument :

1. ID Card
All personel who pass the induction process will get the ID Card. ID card clasiffied as
Employee ID Card and Visitor ID Card. Special ID card will issued for several restricted
area.The worker who will enter to the job site, should have personally project ID card.

2. Gate Pass
Gate Pass is a identification instrument for any vehicle that allow to enter the job site.
All vehicle that will entering the job site shall identified.

3. Log Enter & Log Exit


Is a permit for entering to the job site and taking out from the job site.

4. Fence perimeter and enterance gate


Build for avoid anauthorized people entering the job site. The enterance gate will be
personel and vehicle check.
