SUBJECT : Exemption of Detail in Command Activities, Request for

DATE : June 9, 2022

1. This has reference to the present medical condition of the undersigned

who suffered from bone fractures due to bicycle acccident.

2. Please be informed, that the undersigned underwent major operation on

his right shoulder and find difficulties to move and lift his right arm due to the said
operations. Likewise, the attending medical officer recommends for rehabilitation and
avoid stream movements of his right shoulder. Relatedly, the undersigned is still
undergoing physical theraphy to fully mobilize his right shoulder. (see attached
medical certificate).

3. In this regard, the undersigned is requesting for exemption of detail to any

command activities due to his medical condition.

4. Further request favorable consideration.
