and postal address at Zone 3 Kisolon Sumilao, Bukidnon after having been duly sworn in
accordance with the law hereby depose and say:

That, I am a Senior High School Teacher II of the Department of Education Division

of Malaybalay City with a permanent position, and my employee Identification Number is
4989470;

That, I am applying for 180-day Personal Leave of Absence effective September 1,

2022 to February 28, 2022;

That, the said personal leave of absence needs school and Division clearances;

That, as part of the Division clearance, I attest that Department of Education

Division of Malaybalay City will not be liable for my existing loan at MEMBA, GSIS and BDO-
Network Bank;

That, I verify that I will take responsibility in paying the said loans while I am having
my 180-day Personal Leave of Absence

That, I am aware that I will be administratively and criminally liable for any false
statement of representation made in this document or for any other documents submitted
in connection with this affidavit; and

That, I am executing this affidavit to attest truthfulness, veracity, and due execution
of the foregoing statements and for whatever legal purpose it may serve.

to April 2, 2023;

That, the said personal leave of absence needs school and Division clearances;

That, as part of the Division clearance, I attest that Department of Education

Division of Malaybalay City will not be liable for my existing loan at GSIS;

That, I verify that I will take responsibility in paying the said loans while I am having
my 180-day Personal Leave of Absence
