AFFIDAVIT OF CONSENT AND SUPPORT

I, ERICKA MAY PADUA-LEE, of legal age, Filipino, married to MIN HO LEE, and a resident of
044 BRGY. SAN SEBASTIAN,MATAASNAKAHOY,BATANGAS, after being duly sworn in accordance
with law do hereby depose and say, THAT:

1) I am the mother of DANAYA LEE, who was born on MAY 09,2002 in CHUNG-ANG
UNIVERSITY HOSPITAL,SEOUL KOREA;

2) I am giving my consent for my child to travel to SOUTH KOREA and accompanied by my

sister DAISY MAY PADUA;

3) That I am funding their vacation and am financially capable of supporting them during their
stay abroad;

4) That I guarantee she will not break any existing SOUTH KOREA laws;

5) That I execute this affidavit for whatever legal purpose it may serve in connection with my
child’s application for a passport at the Department of Foreign Affairs and/or travel clearance from
the Department of Social Welfare and Development.
