What Happened?
On September 4, 2019, the IP and two of his colleagues were on their general shift duty. Around 9:30pm,
after completing the shift, they were waiting near the HR Bock for a company vehicle to ferry them till the
main gate. They got into a company car, parked in front of the Occupational Health Centre (OHC), but, the
driver asked them to hire another vehicle.
While getting out of the car, IP’s left ankle got twisted between a parapet & the car (image given below).
He did not report to the OHC about the incident and went home. Next day, after consulting a doctor and x-
ray done, the report revealed fracture on his left ankle. Three weeks of bed rest has been advised.

Preliminary Findings:
1. The car was parked on entry/exit road of parking area and very close to the parapet. It was also found that
the vehicle was not properly parked.
2. Illumination near the incident area was inadequate.
3. The IP was not careful while getting out of the car.
4. The IP did not report to the OHC about the incident.
5. Neither IP nor his colleagues were aware that such incidents need to be reported to the OHC/HOD.

Immediate Actions:
1. The incident was discussed in the daily production meeting. It was communicated that such incidents
need be reported to OHC/ HOD immediately.
2. The incident was communicated to all employees and contract workers in Mass Meeting/Tool Box Talk
to be careful while boarding any vehicle.

Recommendations:
1. Vehicles should be parked at designated area only.
2. Boarding should not be done at improper location.
3. All incidents should be reported to OHC/ HOD immediately.
4. Illumination of the area in front of the HR Block needs to be improved.
5. One should be cautious while boarding/alighting from any vehicle.
