This is to grant permit to the following individual/s to travel to Corro-oy, Santol, La Union on
August 13, 2021. Said individual/s has sought travel permission to come back in this municipality
to attend burial of their deceased relative.

b. Must show NEGATIVE RT-PCR TEST RESULT issued within the last 72 hours
c. Must register in NAPANAM Website (https://npnm.launion.gov.ph/#/regqrpass)
(https://npnm.launion.gov.ph/#/getqrpass).

Moreover, this permit does not give them the right of excuse if they are being stopped at
checkpoints wherein a higher law will prevail.
