AFFIDAVIT OF UNDERTAKING
for Request for Condonation and Writing Off of Unpaid Irrigation Service Fee
(IRR FISA Form 3)

I, ANGEL M. PASON / FERNANDO V. TAGUD,of legal age, Filipino, and a

resident of CURVADA, KAPATAGAN, LANAO DEL NORTE, after having been duly
sworn to in accordance with law, depose and say that:

1. I declare that my landholding (s) covered by the service area of any National
Irrigation System or Communal Irrigation System is:

 Not more than eight (8) hectares in aggregate, regardless of location, and
not necessarily contiguous farm lots;
 Not a corporate farm or plantation;
 Not a fishpond or for other non-agricultural crop use; and
 Not using the irrigation system as drainage facilities.

2. As a qualified beneficiary of Republic Act 10969 otherwise known as Free

Irrigation Service Act (FISA), I am applying for the condonation of all unpaid
Irrigation Service Fee (ISF) attributed to my Iandholding(s) as stated in the
attached Statement of Account (SOA) prepared by the National Irrigation
Administration (NIA) of which I acknowledge to be true and correct;

3. Upon approval of my application for condonation, I am requesting NIA that said

unpaid ISF be written off from the books of said Agency in compliance to Section
4 of FISA;

IN WITNESS WHEREOF, I have hereunto affixed my signature this March 22, 2023 at
