CONSENT TO USE AND DISCLOSURE OF CHILD1’S PERSONAL DATA
I, the undersigned hereby declare that I have read the Multimedia University (“University”)’s Privacy Notice, which is available at www.mmu.edu.my and understand:

i. the content of the University’s Privacy Notice;

ii. that the University will collect, process, store and use my child or legal ward’s (as identified below) (“Child”) personal data and sensitive personal data including
but not limited relating to the Child’s health including any disability (this may include medical records and reports), religious belief and misconduct (if any)
("Personal Data"), which I have provided (or someone provides on my behalf including by my Child) for the purposes of my Child’s enrollment and related
(including but not limited to the processing of medical leave submission, visa process submission and any other purposes which the University will inform me
from time to time) and/or subscription of the University’s products and services and related including in connection with the Child’s attendance at MMU
(“Purposes”); and

iii. that the University will only process, use or disclose such Personal Data in accordance with the University’s Privacy Notice and as permitted under the Personal
Data Protection Act 2010 and/or any laws made thereof

You, the parent or legal guardian of the Child understand that:

If you do not agree to your Child’s Personal Data being provided to the University, then this could impact the enrollment of the Child to the University including continued
provisioning of the products and services provided by the University.
Under the law, you may have the right of access to, and correction of such Personal Data provided. Please contact the University in such circumstances.
In this connection, I, the undersigned hereby consent and authorise the University to collect, use, process and store my Child’s Personal Data for the abovementioned
Purposes and/or in the manner outlined in this form.
