Consent Form

Please read the provisions (“Consent”) on allowing Adamson University and Blackboard to collect,
process, store and shall dispose, personal (“PI”) and sensitive personal information (“SPI”). The word “I”
refers to data subjects providing the data, and consent to the terms of this form.

To proceed with the registration of your child/children to the Adamson’s E-Learning, powered by
Blackboard, we need to obtain your consent, in compliance to the Data Protection Act of 2012, on the
following:

I allow Adamson University to:

A) Collect personal data about me and/or my child/children. The record of which will be stored in
Blackboard’s cloud server and can be accessed by authorized University personnel only.

B) Store the collected personal information for a period of five (5) years.

C) Use the information collected about my child/children only for:

1) Registration to Blackboard;
2) Your child/children to join the online recorded classes;
3) Your child/children to officially complete the modules/course;
4) Submission of any required exams, assignments, projects, quizzes, etc.
5) Other, as required.

D) Share the collected personal data to:

1) Third party platform provider;
2) Internal departments of the University for processing of the registration and completion;
3) Other relevant parties that AdU deems necessary to share the personal data relevant to this
agreement; And in the process expect, that our personal data will be treated with utmost
confidentiality.

I understand and I’m fully aware of Adamson’s Data Privacy Notice, which contains the details of my
rights as a data subject, that can be accessed in the University website.

I understand that Adamson will not process any of the items above, which I have not provided my
consent to. However, I also understand that any or all of the following items may NOT be possible if
consent is not given:

1) For the registration of my child/children to proceed;

2) For them join the online recorded classes;
3) For them to officially complete the module/course;
4) For them to submit any required exams, projects, assignments, quizzes, etc.,
I understand that if there are questions and concerns to this consent clause I will reach out to the
