Good Day!

This is to present my proposal for your Ask Marites App capstone project. Below are the
details.

Deliverables
Ask Marites App is a chatbot app that provides information such as contact details and
addresses of all those who provide clinical services, hospital admission, laboratory services,
diagnostic tests, and therapy available in Daet, Camarines Norte. Its purpose is to create a
platform capable of providing the Daeteños and other people from different parts of
Camarines Norte who come to Daet for clinical services efficiency in finding the right place,
and contact of medical services.

Features:
a Interactive Chatbot - asks query to the user, fetches the needed data from
the database, and displays it.
b Firebase-Supported - an online platform where to upload data for the database, it
also provides key analytics on the app.
 c) About page - displays information about the application

Requirements:

1. Android phone with a minimum android version of 4.4

2. Information on all involved medical services. To be submitted on or before February 15.

Cost:
- Down Payment - ₱4,000 to be paid on or before February 15.
- Final Payment - ₱16,000 to be paid on or before Project Completion

SERVICE AGREEMENT

1. Any bugs/issues report will be fixed by the service provider as long as feature related
and within the agreed scope. It has a one-month free subscription. Any bugs/issues
reported one month after project completion will be charged. Rate will depend on the
scale of the issue.
2. Every modification or additional feature request on the application aside from the agreed
specifications will be charged ₱1,000.
3. The client shall pay the fee on or before the specified date.
