Introduction
The Knowledge Assessment is a set of general and workplace questions testing your
knowledge and understanding of the general theory behind the unit.
You must answer all Knowledge Assessment Questions using your own words. However,
you may refer to your Learner Guide and other relevant resources and learning materials to
complete this assessment.
Some questions cover processes you would likely encounter in a workplace. Ideally, you
should be able to answer these questions based on the processes that are currently in place
in your workplace. However, if you do not currently have access to a workplace, then answer
the questions based on processes that should be implemented in a typical workplace setting.
The Practical Assessment is made up of the Project Assessment. This assessment tests your
practical skills with respect to the requirements of the unit of competency relevant in this
workbook.
The Practical Assessment requires you to complete and submit workplace documents and
other documentation relevant to the unit of competency.
The evidence you submit must be your own work except where due reference is made and
where you are required to submit supplementary workplace documents such as policies
and procedures.
When completing the assessments included in this workbook:
1. Read the instructions provided in each task carefully before attempting to complete
the task. The instructions will guide you on how to answer the question or complete
the task satisfactorily.
2. Follow the steps provided in each task.
 If the question instructs you to describe, provide a description as your
response. If the question instructs you to list, provide a list as your response.
 Where there is a number of required responses, provide the required number
of responses. For example, if you are asked to list three responses, provide
three responses.
3. Ensure that all your submissions for this assessment indicate your first and last name
and that these submissions have been named according to the file naming
convention prescribed in each task.

The Context of Assessment
To complete the assessments in this workbook, students need to have access to their
learning materials and the Internet. The Knowledge Assessment may be completed wholly at
the candidate’s home or chosen place of study. The Practical Assessment must be completed
in a workplace or a simulated environment.

Assessment Methods
This workbook uses the following assessment method/s:
1. Knowledge Assessment
A set of general and workplace questions testing the candidate’s general knowledge
and understanding of the general theory behind the unit.
2. Practical Assignment
A series of written practical tests assessing the candidate’s practical knowledge and
understanding of the unit of competency.

Resources Required for Assessment


The candidate will need access to:
 Computer with Internet, email access, and a working web browser

 A workplace that will allow you access to:
o Workplace supervisor
o At least two individuals who need to be consulted about and participate in
the organisation’s WHS risk management.
Individuals include employees, duty holders as specified in the WHS
legislation (officers, workers, and other persons in the workplace) not
included in the groups below.
o At least two groups who need to be consulted about and participate in the
organisation’s WHS risk management.
Groups include health and safety committees (HSCs), a specific department
or a team in the organisation that does not include the individuals above.

The Work Health and Safety Act 2011 (Qld) requires Department of Education (DoE)
workplaces (including schools) to immediately advise Workplace Health and Safety
Queensland (WHSQ) of notifiable health and safety incidents

Australian Standards and Australian/New Zealand Standards (the standards) are developed by an
independent organisation called Standards Australia. It prepares and publishes voluntary technical and
commercial standards which are sometimes adopted by occupational safety and health regulations.
Specifies requirements for the design, selection, construction and installation of fixed platforms,
walkways, stairways and ladders that are intended to provide safe access to places used by operating,
inspection, maintenance and servicing personnel.

6. AS/NZS ISO 45001:2018 are standards published by Standards Australia for


occupational health and safety management systems.

An OHSMS is a coordinated and systematic approach to managing health and safety risks. ... In doing
so, they establish safer working environments that protect people at work by eliminating, or better
managing, health and safety hazards. The aim of Risk Management is to identify what could go
wrong, i.e. finding what can cause injury or harm to workers, and to decide on proper safety control
measures to prevent work accidents and occupational diseases and implement them (i.e. risk control).

7. AS/NZS ISO 1891.1:2007 are standards published by Standards Australia for


industrial fall-arrest systems including harnesses and ancillary equipment.

a. Person conducting business or undertaking (PCBU)

Their duty of care: A 'person conducting a business or undertaking' (PCBU), who is usually
the employer, has the primary duty of care under the Work Health and Safety Act 2011
(WHS Act) to ensure the health and safety of workers and others at the workplace, so far as
is reasonably practicable. The PCBU is the legal entity carrying out a business or
undertaking and is important to the scheme of the new legislation as it is the primary duty
holder under HSWA. A PCBU is intended to be broad in its definition so that it covers all
work and business activities.

b. Officers

Their duty of care:


Under section 27 of the WHS Act, if a PCBU has a duty or obligation under work health and
safety legislation, an officer of the PCBU must exercise ‘due diligence’ to ensure that the
PCBU complies with the duty or obligation.

The officer’s obligation of due diligence relates to any duty or obligation that the PCBU may
have under the WHS Act or WHS Regulations, and is therefore potentially extensive in
scope.
However, a definition of ‘due diligence’ is provided, which may help an officer in finding
practical ways to comply with the duty. In particular, section 27(5) defines ‘due diligence’ as
including taking reasonable steps:

 to acquire and keep up-to-date knowledge of work health and safety matters
 to gain an understanding of the nature of the operations of the PCBU’s business or
undertaking and generally of the hazards and risks associated with those operations
 to ensure that the PCBU has available for use, and uses, appropriate resources and
processes to eliminate or minimise risks to health and safety from work carried out
as part of the conduct of the business or undertaking
 to ensure that the PCBU has appropriate processes for receiving and considering
information regarding incidents, hazards and risks and responding in a timely way
to that information
 to ensure that the PCBU has, and implements, processes for complying with any
duty or obligation of the PCBU under work health and safety legislation
 to verify the provision and use of the resources and processes referred to in the
three preceding dot points.

32. Explain how the Hierarchy of Control Measures is used in establishing the level of
risk.

The level of risk can be determined based on the Hierarchy of Control Measures
(range of control methods used to eliminate or control hazards in the workplace)
by evaluating the risk against the hierarchy. It starts by looking at the risk and
asking if it can be eliminated, if yes, then the risk is totally eliminated, i.e. there is
no existing risk anymore. However, if it cannot be eliminated, you move to the next
control, which is the substitution – can the hazard where the risk is coming from
being substituted for something that is less hazardous? If yes, then we’ve reduced
the risk somehow, and this goes on down the entire hierarchy until the PPE control.
If the hazard could not be eliminated, substituted, controlled through engineering
adjustments, then it is established that the risk is still there, and the level of risk is
still very high.
Hazards should be re-assessed after the implementation of control measures, to
review if the risk has been reduced enough.

This task requires you to lead one work health and safety hazard identification process
in one work area in your workplace/organisation in compliance with relevant policies
and procedures.
Hazard identification processes may include but are not limited to safety inspections, site
inspections, equipment testing and tagging, WHS consultation, document review (e.g.
historical data on injuries, incidents, and hazards previously reported), process mapping,
etc.
Read the instructions carefully before proceeding.
Steps to take:
1. Locate the WHS Hazard Identification Form in the following pages.
The WHS Hazard Identification Form outlines all the information you need to
document as you lead work health and safety hazard identification processes.

risk mitigation, any forms and templates you are required to complete
during the planning of the risk control, procedures for updating risk register,
etc.
6. Complete Part D of your WHS Risk Assessment Sheet by following the
instructions below.
Part D. Risk Control Options
In Part D of the template, consult with two required stakeholders and work with
them to (for each of the four risks you identified in Task 2.2):
i. Determine three risk control options which can be implemented to
mitigate the risk, and for each:
a. Describe the risk control.
b. Explain how the risk control follows Hierarchy of Control.
c. Describe its strengths.
d. Describe its weaknesses.
At the end of this task, your Part D must document 12 risk control options.
ii. Select the most suitable risk control option that you will implement to
mitigate the risk by ticking the number of the risk control option you
selected.

 Strengths and weaknesses of your risk control options.

iv. Have the two stakeholders you consulted with complete and sign the
Stakeholder Declaration section of Part D. Their signatures must be

demonstrated all the skills outlined in Observation Form 3.2.

3.4 Implementation of Risk Controls

This task requires you to implement two of the four risk treatment plans outlined in your
WHS Risk Assessment Sheet in accordance with organisational policies and procedures.
Read the instructions carefully before proceeding.

3.2.
In this task, you will complete Part E of your WHS Risk Assessment Sheet.

2. Consult with your supervisor regarding opportunities for you to implement two

This task requires you to evaluate the effectiveness of your risk control implementation in
Part 3.
Specifically, you will need to:

 Establish the key performance indicators (KPI) for the risk control implementation.
 Review effectiveness of the for the risk control implementation in accordance with
your organisation’s WHSMS and WHS legislative requirements.

Steps to take:
1. Locate the WHS Risk Management Evaluation Report template in the following
pages.
The WHS Risk Management Evaluation Report template outlines the information
you need to gather and document to evaluate the effectiveness of your risk
control implementation in Part 3.
You are to complete one report for each of the two risk controls you
implemented in Part 3. At the end of this task you should have two completed
and signed WHS Risk Management Evaluation Reports.
2. Access and review your organisation’s Work Health and Safety Management
System (WHSMS) to determine:

evaluation for this task.
3. Access and review relevant WHS legislative requirements for evaluation of risk
management processes.

controls you implemented in Part 3. Follow the instructions below.

which the evaluation will be done.
vi. Indicate the risk control implemented which will be evaluated.
vii. Outline at least one key performance indicator (KPI) applicable to this risk
control implementation.
Key performance indicators (KPIs) include but are not limited to measures
of success, metrics that will be used to assess the risk control
implementation.

stakeholders in accordance with your organisation’s requirements for communicating of
risk management evaluation findings.
These three stakeholders must be organisational personnel whom you are required to
report the selected risk controls to, as per organisational policies and procedures.
Stakeholders you meet with in this task may include but are not limited duty holders as
specified in the WHS legislation (officers, workers, and other persons in the workplace,
Health and Safety Representatives (HSRs), Health and Safety Committees (HSCs), a
specific department or a team in the organisation that is afected by the hazards and
risks you identified in Task 2.1 and Task 2.2.
