Although the Contractor has provided stands/frames for electric cables but still they are found inside water. There is major hazard of electrocution of whole area. Moreover, they pose slip and trip hazard, Power lines have been separated from the water lodges area. ‘At Outlet area, workers are fixing hooks for wire mesh at more than 2m height. No platform or ladder is used and workers are climbing directly on the rock. Fall hazard exists which may cause severe injuries. Ti order to prevent such unsafe activities on the site, we have invited all the Chinese in-charge to the conference room and in the presence of all the participants, they have been informed about the unsafe activity on the site and they have been strictly instructed find a good way to enhance the activity at site safe and sound do not endanger any workers. provide safe ladder to work Photo 1at height should be as per the standard, scaffolding should be as per the standard. Shoterete is in progress at Outlet slopes, Only 2 of 6 workers are having safety goggles while they all are lacking other mandatory PPE. The | said platform is absolutely not safe in all manners. Appropriate scaffolding has been erected. All standard precautions have been taken in scaffolding. The following steps are taken: Base plate, triangle, wooden planks all planks are tied, top rail, mid rail, Toe board, proper ladder with handrail, Inclined support.Workers are observed in loose dress at workplace. It was said that they are newly hired and the worker put his shirt inside his shalwar. Loose dress has hazard of nip point and loose parts of the dress may get entangled in rotatory parts of machinery. Uniform has been provided to the mentioned worker. Further, the in-charge of grouting section is strictly instructed not to allow any body to enter the site in loose clothing, first provide them with uniform and then allow them to center the site. Drilling machines for grouting has been observed fixed near main access road at Intake site. Barricades are provided only with safety cones and safety jersey barriers. ‘The drilling machine has been removed from the mentioned location.Chinese worker has been ‘observed working in rotating machinery. Its rotatory parts may have nip point the naked hand. Furthermore, working without safety gloves make the worker more vulnerable to hazards. Safety gloves have been provided to the workers 8. | At Intake site, the tanker of the cement comes for | grouting section after in every two or three days. The vehicles and the drivers | were private. When emptying the tankers at site, during this process, they have not any PPEs. Such as safety helmet and reflective hi vest. "The matter has been discussed with the in- charge grouting team and they have been instructed not to allow any body to enter the work site without PPE. The electric cables have been removed from the water and additionally the site electrician has been informed about the proper insulation of all the electric cables in the work place area. All electrical cables are inspected by the PCL HSE inspector, if he observes any damaged cable or weak insulation, So it is taken into the notice of the electrician.‘At Intake site, Damage welding cable found with welding plant of grouting section on site. The excavator could trip over and crash. Or even the swinging arm of the machine could hit somebody. This raises the safety concem, ‘A flagmen has been appointed with the excavator machine, 19. ‘At penstock back slopes, contractor using unsafe scaffold structure for placing the shot concrete on slope.
