Re: Notice – Recklessness, Undertime

Mr./Ms.

On your interview and orientation, you were duly informed that among your duties
stipulated in your contract is to keep the shop (Station) safe from fire hazards by observing due
diligence reasonably expected of you, particularly under Clause “2.2.g” thereof. Moreover, you
were duly informed that you are required to be on duty for eight (8) hours, and even to extend
up to twelve (12) hours if necessary. This was likewise stipulated under Clauses “2.2.b”, “4.1”,
and “4.4” of your Probationary Contract.

On December 12, 2021, at around 3:00 P.M., you claimed to have electrocuted your
right hand with one of the station’s power outlets. You further alleged that the pain in your
right hand and forearm has become unbearable and that you could not move it properly. As
such, you requested to go on undertime at 4:00 P.M.

On the same day, upon further inquiry and investigation, you admitted to have acted
out of recklessness in touching the power outlet with a wet hand. You knew fully well that such
action could cause you severe injury or death, and even endanger the lives of others as well as
the entire building from catching fire. It is fortunate that you only got out with nothing more
than a stung arm without any apparent or long-term injury. However, the danger of your
recklessness / gross negligence cannot be understated.

Your action of intentionally touching a power outlet with a wet hand thereby inflicting
upon yourself an alleged injury, which resulted in your reduced working hour and thereby
negatively affecting the Station’s business operation is a clear act of recklessness and/or gross
negligence. What is worse- you endangered your limb and life, the life of the other occupants
of the building, and we need not mention the properties involved.

You are hereby notified that this incident shall substantially affect your score in your
monthly evaluation for your Probationary Employment Status.
