The  Interprofessional  Approach

Who  needs  an  interdisciplinary  team?  
•  Pa5ents  with  demen5a  typically  have  complex  
medical,  psychological  and  social  needs.  
•  Teams  are  more  effec5ve  in  assessing  pa5ent  
needs  and  crea5ng  a  comprehensive  plan  than  
when  each  professional  works  alone.  
•  Family  caregivers  also  benefit  since  their  
strengths  and  needs  can  be  incorporated  into  
the  plan  of  care.
