ADMINISTERING PARENTERAL MEDICATIONS

A. ASPIRATING A SOLUTION FROM A VIAL Need Satisfactory Comments

Practice
1. Wash hands.

2. Assemble/prepare syringe with g.18-19 needle, vial of solution, cotton balls


moistened in 70% alcohol.
3. Clean rubber cap of vial with cotton balls moistened in 70% alcohol.
Discard the used cotton ball in appropriate receptacle.
4. Hold the syringe. Remove needle cap and place it on hypodermic tray.

5. Draw air into the syringe equivalent to the amount of solution to be


aspirated from the vial.
6. Insert needle firmly to center of rubber cap of vial.

7. Invert the vial, inject the air into the vial.

8. Aspirate the desired solution.

9. Remove the needle from the vial.

10. Expel the air bubbles from the syringe slowly and carefully.

11. Recap the needle and place it on hypodermic tray between 2 layers of
sterile towel.
12. Identify the syringe with a medication card.

13. Transport carefully to the patient.

B. ASPIRATING SOLUTION FROM AN AMPULE

1. Verify physician’s order with the medication card.

2. Wash hands.

3. Prepare appropriate syringe – needle unit.

4. Take the correct ampule of solution, check and tap stem to remove
trapped medicine.
5. Take a sterilized gauze, cover the stem of the ampule and break it off by
pushing outward (file the neck of the ampule if necessary to start a clear
break).
6. Place the open ampule on the working table.

7. Take the syringe-needle unity remove the needle cap.

8. Guide the needle into the ampule, take and invert the ampule while
aspirating the ordered solution or medicine.
9. Remove the needle from the ampule, expel air bubbles. Replace needle
care.
10. Replace needle cap.

11. Check for the correct medication.

12. Place the prepared syringe of medication on a hypodermic tray between


layers of folded sterile towel.
13. Identify prepared syringe of medicine with the medication card.

14. Transport safely to the patient.

1. Wash hands.

2. Assemble/prepare solution, tuberculin syringe, dry and moistened


cotton balls in 70% alcohol and hypodermic tray.
3. Aspirate ___ distilled water as is A above.

4. Aspirate___________ solution using sterile technique.

5. Allow the injection to mix by moving the syringe gently, keeping the
plunger in place.
6. If aspirating needle is used to prepare solution, change the needle into
g. 25. Avoid touching the top of the barrel of the syringe, and the inside
of the hub of the needle.
7. Keep the prepared solution inside the layer of the folded sterile towel of
the hypodermic tray.
8. Transport carefully to the patient.

9. Identify the client and inform him of procedure. Assist him to sit
comfortably on a chair or lie down on bed.
10. Expose the inner aspect of the forearm. Choose site with no visible veins,
scars, and lesions.

11. Clean the site with alcohol-moistened cotton balls, in circular motion, from
inside going outward. Discard the cotton balls in appropriate receptacle.
12. Remove the needle cap and place it on the hypodermic tray.

13. Place your non-dominant hand under his arm and keep the skin of cleansed
site taut by stretching the skin with your thumb and fingers. If client has a
big arm, stretch the skin of the site over the arm. Ensure that you do not
touch the cleansed area.
14. Insert the needle almost parallel to the skin (10-15 degrees), with bevel
facing upward.
15. Inject the solution slowly until a wheal is formed.

16. Withdraw the needle. Wipe the area with dry cotton ball.

17. Encircle the wheal with a pen and indicate time of administration.

18. Observe the site for wheal enlargement, reddening, itchiness within or
after 30 minutes.

19. Document the time, medication tested, site of injection, and your
observations.

Practice
1. Wash hands.

2. Assemble/prepare equipment and medication.

a. Draw the solution into a sterile syringe using sterile technique. See to it
that there are no air bubbles inside the syringe.
b. Recap the needle and place syringe inside the layer of sterile towel of
hypodermic tray.

3. Identify client and inform him of procedure.


4. Assist him in a comfortable in sitting or lying position.

5. Expose the deltoid muscle (1-1.5” from shoulder). Choose a site which
has no signs of skin irritation, lesions, swelling, or scars.

6. Clean the site with cotton balls moistened in 70% alcohol, wiping in
circular motion, and start at the center going outward. Allow the skin to
dry.
7. Hold the syringe and remove the needle cap. Expel air bubbles if present.

8. With your non-dominant hand grasp the deltoid site firmly, using the
thumb and index finger, keep the skin taut. Holding the syringe with
needle at 90 degree angle (like holding a pen), pierce the skin quickly, and
thrust the needle into the muscle.
9. Aspirate by holding the barrel of the syringe steady with your non-
dominant hand and by pulling back on the plunger with your dominant
hand. If blood appears in the syringe, withdraw the needle, discard the
syringe, and prepare a new injection.
10. If blood does not appear, inject the medication slowly and steadily.

11. Withdraw the needle quickly, and massage the site.

12. Discard the used syringe with needle into proper receptacle.

13. Document all relevant information.
