• Breif Description : Free quality critical care for low income family.

• Eligibility criteria : Family living below the poverty line upto Rs.1.5 lakhs in
a year.

→ Driving License
→ Aadhar Card
→ Voter ID
→ Handicap Certificate
→ Nationalized Bank Passbook with Photograph
→ School/College ID
→ For urban areas, Government Local bodies/Tahsildar’s stamps and signatures on the
applicant’s photograph.
→ For rural areas, Tahsildar’s stamp and signature on the applicant’s photograph.
→ Passport
→ Freedom Fighter ID Card
→ Senior citizen card issued by central and state government Of India
→ Defence ex-service card issued by Sainik board
→ Any photo ID proof issued by Govt. of Maharashtra/ Govt. of India.

• Full description of scheme : Mahatma Jyotiba Pule Jan Arogya Yojana is a
flagship health insurance scheme of Government of Maharashtra. The
scheme provides end to end cashless services for identified diseases
through a network of service providers from Government and Private
sector. The scheme earlier was known as Rajiv Gandhi Jeevandayee Arogya
Yojana which was started from 2nd July,2012 in eight districts and then was
expanded to 28 districts of Maharashtra from 21st November, 2013

(AB-PMJAY)
(Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (PMJAY)
was launched by Government of India from 23rd September,
2018. AB-PMJAY was launched in the Maharashtra in integration
with Mahatma Jyotiba Phule Jan Arogya Yojna and was
implemented on mixed Insurance and Assurance Mode.)
