AFFIDAVIT OF GUARDIANSHIP

I, ANACITA O. GABRIEL, of legal age, married, Filipino, and a

resident of Brgy. Guibwangan, Gamay, Northern Samar, after having been
duly sworn to in accordance with the law, hereby depose and say:

1. That I am the paternal grandmother of the minor, Christian L.

Gabriel, who was born on September 3, 2005, in Brgy.
Guibwangan, Gamay, N. Samar.

2. That since he was just seven years old, Christian L. Gabriel

has already been under my care and custody as his father
died in 2013 and his mother has already contracted another
relationship with a man and has now a separate family.

3. That the care and help that I used to extend to Christian L.

Gabriel are out of my love and affection for him being my
grandson; hence, I have been providing him moral,
psychological, and financial support.

4. That I am not otherwise disqualified by law to act as her

guardian;

5. That I executed this affidavit to attest to the truth of the

foregoing statement and for whatever legal purpose this
may serve.

IN WITNESS WHEREOF, I hereunto set my hand and affixed my

signature this 3rd day of September 2021, in Gamay, Northern Samar,
