JOINT AFFIDAVIT

We, MILAGROS O. LOCABA and APOLONIA O. TUBERA, both of legal age,

Filipino, widow and residents of Barangay 8 Poblacion, Catubig, Northern Samar,
Philippines, after having been duly sworn to in accordance with law, do hereby depose and
state the following:

1. That we know the person of ROMEO ESTORES TURLA because he is our

relative;

2. That said ROMEO ESTORES TURLA died on February 5, 2014 at Catubig,

Northern Samar due to heart attack;

3. That said fact of death is not yet registered in the Local Civil Registrar of Catubig,
Northern Samar or in any other Local Civil Registrars Office up to this date;

4. That we are executing this Joint Affidavit to attest to the truthfulness of the
foregoing and for the purpose of belatedly registering the death of ROMEO
ESTORES TURLA and for whatever legal purposes that this Joint Affidavit may
serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 18 th day of June
2017, in the Municipality of Catubig, Northern Samar, Philippines.
