AFFIDAVIT

I, JOWELA LUZON VEDRA, Filipino, of legal age, single, and a resident of #0164 Maximo
Lasco St., Pooc Occidental, Tubigon, Bohol, after having been sworn in accordance with law, hereby
depose and say:

1. That sometime on May 2007, I applied for a Non-Professional Driver’s License with the Land
Transportation Office, Tagbilaran District Office, Tagbilaran City (LTO-Tagbilaran);

2. That my said application was approved by LTO-Tagbilaran;

3. That my Non-Professional Driver’s License, which was supposed to be valid until June 8,
2009, was not issued to me due to my own fault as I was not able to have my picture taken by LTO-
Tagbilaran because I suddenly went to Manila at the scheduled time for taking my picture;

4. That by these presents, I hereby undertake to comply with all requirements of LTO-Tagbilaran
for the issuance of my Non-Professional Driver’s License and its subsequent renewal;

5. That I execute this affidavit voluntarily to attest to the truth of the foregoing statements to
support the renewal of my Non-Professional Driver’s License with LTO-Tagbilaran;

6. That further, I execute this affidavit for whatever legal purpose this may serve.
