AFFIDAVIT

I, ELMERSON M. LEGARSE, Filipino, of legal age, married, and a

resident of Catubig, Northern Samar, after having been sworn according
to law, hereby depose and say:

1. That I was the previous owner of Dhyvens Bakery located at Brgy.

4 Poblacion, Catubig, Northern Samar;

2. That I bought the said bakery, including its tools and equipment,
from Ma. Victoria Tenedero on 15 June 2011;

3. That the said bakery was previously owned by said Ma. Victoria
Tenedero operating under the name of Marivics Bakery;

4. That sometime on May 2015, I subsequently sold the said bakery

and its tools and equipment to Fred R. Celajes;

5. That at present, the said bakery is owned by said Fred R. Celajes

and is operating under the name of Pan de Catubig;

6. That I am executing this affidavit to attest to the truthfulness of

the foregoing statements and for whatever legal purpose this may
serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this

20th day of February 2017, at Catubig, Northern Samar.
