Dear Sir/Madam:

I am personally requesting for a single entry visa for myself. I am CESAR

NORMAN YRUMA CALAMAY, bearer of Philippine Passport No.
EC4926261, and resident of 56 B. Orsolino Street, Barangay 3 Poblacion,
Catubig, Northern Samar. I am a lawyer by profession and is currently employed
as Clerk of Court VI of Regional Trial Court, Branch 38, Gamay, Northern
Samar, under the Supreme Court of the Philippines.

I am writing to your office to let you know that I am lacking the required latest
Income Tax Return (ITR) BIR Form 2316 for the period covering 12 February
2018 – 31 December 2018. I just got employed in the Supreme Court last 12
February 2018 and the latter has yet to release my ITR for the abovementioned
period as of this date. Nonetheless, as proofs of my income, I have attached
herewith photocopies of my latest available pay slip for January 2019, my ITR
for the year 2017 and my ITR covering the period of 01 January 2018 to 11
February 2018 issued by my previous employer, Department of Education,
Division of Northern Samar, Catarman, Northern Samar.

I will be flying to Japan on 08 April 2019 and will stay until 13 April 2019 for
vacation. I guarantee that I will not become a public charge or burden during my
stay in Japan and will strictly follow all rules and regulations.
