I. Composition of Post-Qualification Team

II. Purpose of Post-Qualification

The team conducted a post-qualification on the subject bidder the Thirdy

Construction & Supply with office address located at P-3 Brgy. 4, San Francisco,
Agusan del Sur. The purpose of the post-qualification were to verify the original
copies of the documents submitted during the opening of bids and to determine the
capacity of the said bidder to supply and deliver the agricultural inputs stated in the
Bid Bulletin Issued by the Institute.

III. Highlights of the Post-Qualification

The team was headed by BAC chairperson Violeta T. Laguna who also
presided the meeting which started at 9:00am in the morning. She then introduced
the Matt Andrew Baquiano, BAC member, and the TWG members who were present
during the activity. She also discussed the purpose and importance of post-
qualification as vital part of the procurement process. Mr. Baquiano evaluated and
discussed the technical aspects of the procurement to

The BAC member said that per evaluation of the necessary documents of the
Post-Qualification team, Thirdy Construction & Supply was in conformance to the
institute requirements and criteria for post-qualification.

The team was able to verify the original eligibility documents from that of the
copies submitted during the bid opening.

After the evaluation, Ms. Laguna then informed the subject bidder that after
the evaluation, the team will prepare the post qualification report and advised them
for further notification.

SUBJECT : POST QUALIFICATION OF PROCUREMENT FOR THE CONSTRUCTION OF

CONSTRUCTION & SUPPLY

The team conducted a post-qualification on the above subject bidder with office address at P-3,
Brgy. 4, San Francisco, Agusan del Sur. The purpose of the post-qualification was to verify the
original copies of the documents submitted during the opening of bids and to determine if the
said bidder can provide the specified items stated in the bidding documents.

The team was able to verify the original eligibility documents submitted during the bid opening
per checklist attached.

Meanwhile, the team also visited the bidder’s ongoing site project at Prosperidad National High
School located at Poblacion, Prosperidad, Agusan del Sur.

Moreover, the team found out that the above-mentioned bidder complied with all the
requirements and has the capacity to supply and deliver the veterinary supplies specified in
the bidding documents.

RECOMMENDATION:

The team recommends to award the PROCUREMENT FOR THE CONSTRUCTION OF SINGLE-
STOREY SWINE AI SA BARANGAY (SWAIB) PROJECT to THIRDY CONSTRUCTION & SUPPLY.
