About The Advanced Medical And Medical Forum (AMMF)
And The Purpose Of Non-Profit Organization (JIMCA) establishment

Last year, July 24, 2018, Japan Medical Instruments Co., Ltd. (JMIC) has organized the 1st Advanced Medical
and Medical Forum at Hotel Mulia, Jakarta, Indonesia.

JMIC Co., Ltd. is a company which has a business in import/export and also manufacture and sales of
medical instruments. the exchanges with the Republic of Indonesia began in 2015, and the exchange's
activities with medical doctors and medical staffs in Indonesia are going often as time went by, it became
possible for us to understand Indonesia's medical information and the environment surrounding them
gradually.

In July 2018 last year, we recalled our company establishment idea, which is "Contributing to the healthy
society and improving the quality and services of healthcare facilities, medical institutions, and welfare
facilities, etc.", we organized the "Advanced Medical and Medical Forum" in Jakarta, Indonesia. we Invited
famous doctors and experts specializing in cardiovascular surgery professor, neonatal intensive care
professor, director of the Ministry of Health, Labor and Welfare, DMAT director of the Ministry of Health,
Labor and Welfare, honor professor hospital construction specialists from Tokyo University, medical
management consultant, and etc. Furthermore, the forum was supported by the Embassy of Japan in
Indonesia and the Ministry of Economic, Trade, and Industry of Japan and also the government institution in
Indonesia by the opening speech from each institution with the main participants were the hospital group
of State-Owned Enterprises Hospital.

we will continue making every effort to promote Japan medical Instruments and related devices that have
not been introduced to Indonesia or ASEAN countries. As a future step, I would like to make an effort to
build strong cooperative relations between the Republic of Indonesia and Japan with training and
exchanges program of the doctors, nurses, and all related parties of healthcare workers to improve the
medical equipment made by Japan, and also the improvement of maintenance skills of the instruments,
and etc.

However, to organize the medical forum every year and to conduct the medical exchange collaboration
program by our self (JMIC, Co., Ltd.), as a private company we face a big wall and we have our limitation to
realizing it. Therefore, in regard to these activities, in a wide range of our businesses and activities that we
have done so far, and at the same, in order to continuously promote in the ASEAN region, we believe that
corporatization is enabling the development of the medical industry and it is the first step to recognize
Japan contribution in the ASEAN region, and it will strengthening international relation and smoothing
international exchange of medical and medicine.

From now on, we believe that we will continue to organize the "Advanced Medical and Medical Forum" at
the Japan-Indonesia Medical Cooperation Association, which we have established.

The Japan-Indonesia Medical Cooperation Association (JIMCA) is a


Non-Profit Organization for the cooperation and collaboration
between Japan and the Indonesian Republic through the Knowledge
and advanced medicine that Japan has.

The purpose of its NPO is to give a contribution to the improvement of


the quality of healthcare facilities, medical institution, welfare facilities,
and etc for the of both citizens health of Japanese and the Indonesian

This Year`s 「Advanced Medical and Medicine Forum」 is organized by a JIMCA ( Japan - Indonesia
Medical Collaboration) as a new NPO (Non-Profit Organization) established by JMIC Co., Ltd. which was the
organizer for the last year forum (AMMF 2018). The forum`s topic is 「 The Diagnostic Technology of AI and
Radiographic Image Technology to Improve Diagnostic Interpretation Capabilities 」. And There are 3
distinguished specialist medical doctors from Japan will give lectures on its forum.

It is as a continuous act of the forum last year (AMMF 2018), the “Advanced Medicine and Medicine Forum”
aims to strengthen the relationship between Japan and The Republic of Indonesia in medical cooperation,
business exchanges, and exchange knowledge of healthcare worker in Japan dan healthcare worker in

The Cutting Edge of CT and MRI.


- a simple explanation –

What is our job? To find out the essence using X-ray, Magnet or Ultrasound.
Long history & many ways to find out the essence of matter has existed. The
first left anterior oblique view in history was taken by Leonardo da Vinci. We are taking
usually to find out more detail character of the lesion using X-rays. His success was
succeeded by many painters. CT is able to see from 360 degree of body. Multi
detector CT can see from more view points. Pablo Picasso have had the same view
points from a hundred years ago.
Super realism has been achieved by Nederland painters in 15c. by invention of
oil painting. Recent CT has just got ultra high resolution by invention of imaging
detector and low dose. I will show you many clinical cases and differences with
conventional CT.
The same wave has come in MRI using AI technique, deep learning
reconstruction. Many MRI cases with ultra high resolution in short imaging time will be
also shown.
Last topic of my lecture is the ability of computer aided diagnosis. I will show
you our study about computer aides diagnosis/detection (CADx vs. CADe ) system. In
my opinion , CADe system should be more helpful in many hospitals soon. However,
diagnosis by computer will be impossible even in future. They cannot analyze the
essence of the lesion.
I will insist next message as a result, development of imaging machine is
important for our job and patient health, however, most important matter for the
general radiologist should be to master the basic imaging anatomy and the pathology.

The Clinical of CT and MRI Imaging


CT is usually first choice of modality in a wide variety of clinical situations, especially in
emergency room. In acute abdomen, unenhanced CT plays a role in establishing diagnosis including
acute appendicitis, pancreatitis, intestinal perforation, biliary obstruction by cholelithiasis,
ureterolithiasis, etc. The combined use of contrast media is sometimes essential for diagnosing
vascular events, including pulmonary embolism, renal infarction, dissection of the arteries, since
these conditions are usually difficult to diagnose only on unenhanced CT.
Dynamic contrast enhanced CT can be performed by rapid bolus injection of contrast media,
and can provide information concerning the arterial flow. In emergency, arterial phase allows
diagnosis of dissecting aneurysms. In case of hemorrhage, the site of hemorrhage can be detected by
recognition of the leakage of the contrast media. The comparison of the degree of enhancement
enables evaluation of the vascularity of the tumors, and thus allows specific diagnosis of some
tumors. Tumors of high vascularity, such as hepatocellular carcinoma, renal cell carcinoma of clear
cell type, are characterized by strong enhancement on the arterial phase and wash-out on the
venous phase. On the other hand, tumors, tumors with abundant interstitial spaces are characterized
by gradual enhancement. Intratumoral necrosis or cyst are demonstrated as unenhanced areas.
The relationships of tumors and vessels are also important hallmarks. If the tumor vessels
drain into the ovarian vein, the tumor originate from the ovary. If it drain into the mesenteric vein,
the tumor will be gastrointestinal stromal tumor. When the vessels penetrate the tumor, malignant
lymphoma is suspected.
MR images can provide good tissue contrast without use of contrast media, and detailed
anatomic information especially in the brain, spine, pelvic region and musculoskeletal system.
Recently prevailing technique, diffusion-weighted images are inevitable for diagnosing acute brain
infarction. This technique has an excellent potential to depict highly cellular lesions. This potential is
commonly applied to evaluate prostate cancer, and also to a wide variety of malignancy. In this
lecture, I would like to demonstrate clinical application of CT and MR images, discussing the
advantages and limitation of these modalities.

Importance of standardization in emergency and disaster


medicine and further attempts of improvement

Standardization of emergency and disaster medicine is progressing rapidly.


Recently, standardization has been conducted in wide variety of medical fields. In
emergency medical fields, Preventable Trauma Death (PTD) occupied 40% of all
the trauma. However, after standardization of both prehospital and hospital
trauma care such as Japan Prehospital Trauma Evaluation and Care (JPTEC) and
Japan Advanced Trauma Evaluation and Care (JATEC), PTD improved to below
15%.
For disaster medial fields, same improvement has been trying to achieve. In
1995 Great Hanshin-Awaji Earthquake report, Preventable Disaster Death (PDD)
was counted approximately 500 casualties. To prevent PDD, triage and medical
post care was standardized later.
With the aim of further improvements in trauma treatment, use of Doctor
Helicopter and utilizing Hybrid ER have become popular in Japan. Doctor
helicopter started operation in 20011, and rapidly spread to all over Japan and
now 52 helicopters are in operation. With the introduction of the doctor
helicopter, the lifesaving rate, especially in remote areas has major possibility to
increase2.
Hybrid ER is a first medical room that allows treatment and diagnosis
simultaneously. 11 hybrid ERs have been introduced nationwide. Hybrid ER is
important not only in terms of hardware but also in terms of software such as
medical care by the team. Among medical team, radiology doctor and radiologists
play a major role. We introduce what kind of team medical care is necessary by
the movie.

るための健康寿命の延伸は喫緊の課題とされる。
そうした中で、ヘルスケア分野ではカルテ、診療に係る明細書(レセプト)、処方箋、
レントゲン画像、健診結果、服薬履歴、アレルギー診断等に加えて、個人の健康診断・
服薬指導結果、さらには遠隔診断・手術、地域における救急医療時の情報連携や母子の
健康支援等の国民の健康を支えるためのデータを横断的につなげる、いわゆる“一気通貫
の医療システム”の構築が国是となっている。
そこで、日本政府は国家の危機管理や減災対策の観点も含めてAIやIoT、オンライン診
療、マイナポータルを活用したPHR(Personal Health Recordサービス)、そして
Society5.0の取組みを目指す。くしくも第2回高度医療・医学フォーラムはその具体的な
試みを紹介するもので、願わくば人口規模が日本の2倍で津波や地震の災害対策も共通し
た喫緊のテーマとなっているインドネシア関係者との建設的な協議の「場」としたい。
