AFFIDAVIT OF WAIVER AND QUITCLAIM

I, LOURDES B. TICZON, Filipino, of legal age, married, and a resident of
Sindangan, Zamboanga del Norte, under oath, hereby deposeth and sayeth:

That I used to be an employee of Sindangan Farmers and Teachers Multi-

Purpose Cooperative [formerly Sindangan FACOMA, Inc.], at Sindangan, Zamboanga
del Norte.

That I already voluntarily retired from FACOMA.

That I voluntarily waive, relinquish and abandon whatever rights I have over
my retirement benefits worth P118,434.00 in favor of the Sindangan Farmers and
Teachers Multi-Purpose Cooperative [formerly Sindangan FACOMA, Inc.] , at
Sindangan, Zamboanga del Norte.

That pursuant to this waiver and quitclaim, I hereby authorize the Sindangan
Farmers and Teachers Multi-Purpose Cooperative [ formerly Sindangan FACOMA,
Inc.] at Sindangan, Zamboanga del Norte to withdraw, collect, demand, and I encash
whatever check or treasury warrant representing my said retirement benefits, and to
apply the same for whatever obligations I have with them.
