Thank you for seeing Ms. Shauna Mendes, a 25-year-old software developer, who has been recently
diagnosed with bacterial keratitis.
Ms. Mendes, who is single, has myopia in both eyes (rt eye :-2.5 & lt eye: -4). She is allergic to
dexpanthenol eye drops.
Ms. Mendes has been my patient for three years. She presented on 19/03/2022 complaining of a three-
day history of profuse tearing and foreign body sensation in the right eye associated with redness, pain
and difficulty in opening her eyelids after she had worn an old contact lens. Penlight examination showed
corneal inflammation, scarring and rashes. Consequently, tobramycin 14mg/dl eye drop hourly and
cefazolin 50mg/ml eye drop hourly were instituted. Additionally, a tear sample collection for bacterial
examination was ordered.
Today, Ms. Mendes's test result was positive for Pseudomonas bacterial infestation which confirmed the
diagnosis of bacterial keratitis. Accordingly, prednisolone acetate 1% six hourly was prescribed along with
her previous medications. Avoiding contact lenses, eye straining, splashing water on the right eye until Commented [OET1]: and
complete recovery was advised.
Given this history, I am referring Ms. Mendes into your care for further management.
