WHAT IS THE MI SHOT TO WIN SWEEPSTAKES?

The MI Shot To Win Sweepstakes is a lottery-style raffle that gives vaccinated Michiganders a
chance to win more than $5 million in cash and college scholarships.

The goal of the public outreach campaign, consisting of a series of statewide drawings, is to provide
incentives to Michiganders to get a COVID-19 vaccination as soon as possible this summer to get
the state fully back to normal. The aim of the MI Shot To Win Sweepstakes is to increase the state’s
vaccination rate by roughly 9% – or approximately 767,000 Michiganders – to achieve the statewide
goal of 70% of residents age 16 and older who are immunized.

The sweepstakes giveaway is powered by Meijer in partnership with the Michigan Association of
United Ways and the State of Michigan. The statewide vaccine lottery is supported by Blue Cross
Blue Shield of Michigan, Michigan Chamber of Commerce, Michigan Association of State
Universities, Small Business Association of Michigan, Business Leaders for Michigan, the
Protect Michigan Commission and many more.

WHEN DOES THE MI SHOT TO WIN SWEEPSTAKES START?

The MI Shot to Win Sweepstakes (the “Sweepstakes”) begins on July 1, 2021, at 12:01 a.m. Eastern
Time (“ET”).

WHEN DOES THE MI SHOT TO WIN SWEEPSTAKES END?

The MI Shot To Win Sweepstakes ends on Aug. 3, 2021, at 11:59 p.m. ET (the “Sweepstakes Period”).

WHAT ARE THE MI SHOT TO WIN PRIZES?

$1 Million Drawing. There is one (1) $1M Drawing prize for Michiganders age 18 and older
consisting of $1 million awarded as a check or electronic transfer to the confirmed winner.

$50,000 Daily Drawings. There are thirty (30) $50,000 Daily Drawing prizes available for
Michiganders age 18 and older, each consisting of $50,000 awarded as a check or electronic transfer
to the confirmed winners.

$2 Million Drawing. There is one (1) $2 Million Drawing prize for Michiganders age 18 and older
consisting of $2 million awarded as a check or electronic transfer to the confirmed winner.

Scholarship Drawing. There are nine (9) Scholarship Drawing prizes available for Michiganders
between ages 12-17. Each Scholarship Drawing prize consists of a four-year Michigan Education
Trust Charitable Tuition Program contract (valued at $55,000) that may be used to pay for tuition
and mandatory fees at a college or university in accordance with MET terms and conditions. For

HOW DO YOU QUALIFY TO ENTER THE MI SHOT TO WIN SWEEPSTAKES?

Michiganders can qualify to enter the sweepstakes by getting vaccinated for COVID-19 at Meijer or
receiving their shot from their doctor, local health department, pharmacy, hospital, etc. 

WHO IS ELIGIBLE? AND WHEN ARE THE DRAWINGS?

The Sweepstakes consists of the following drawings and eligibility criteria:

a. $1 Million Drawing: Open to all eligible persons age 18 and older who have received at least
one (1) dose of a COVID-19 vaccination between Dec. 1, 2020, and July 10, 2021, and enter
the Sweepstakes as of July 10, 2021. The $1 Million Drawing will take place on or about July
11, 2021.
b. $50,000 Daily Drawings: Open to all eligible persons age 18 and older who receive their first
dose of a COVID-19 vaccination on the date corresponding to the $50,000 Daily Drawing. In
the event that there are no eligible entries received for any given $50,000 Daily Drawing or the
prize cannot be awarded for any reason, the prize associated with that day’s $50,000 Daily
Drawing will be added to the $2 Million Drawing prize.
c. $2 Million Drawing: Open to all eligible persons age 18 and older who have received at least
one (1) dose of a COVID-19 Vaccination between Dec. 1, 2020, and July 30, 2021, and enter
the Sweepstakes as of Aug. 3, 2021. The $2 Million Drawing will take place on or about August
4, 2021.
d. Scholarship Drawing: Open to all eligible persons between ages 12-17 who have received
at least one (1) dose of a COVID-19 vaccination between Dec. 1, 2020, and July 30, 2021,
and enter the Sweepstakes as of Aug. 3, 2021. The Scholarship Drawings will take place on
or about Aug. 4, 2021, and the winners will be announced at a date to be determined. Each
Scholarship Drawing prize consists of a four-year MET Charitable Tuition Program contract
(valued at $55,000) that may be used to pay for tuition and mandatory fees at a college or
university in accordance with MET terms and conditions.

THEIR MINOR CHILD?

Yes. A parent or legal guardian is required to submit an entry for a minor. After entering the minor’s
name and date of birth, you will be prompted to enter the parent or legal guardian’s information. The
entry form for Michiganders 12 to 17 years-old must include a parent or legal guardian’s information,
including their name, address, phone number and email address.

HOW DO I ENTER THE MI SHOT TO WIN DRAWINGS?

There are two (2) ways to enter the Sweepstakes:

a. ONLINE: To enter online, visit the website MiShotToWin.com (the “Website”), complete the
entry form with the information requested, and follow the on-screen instructions to submit your
entry and receive one (1) entry into the Sweepstakes. All requested information on the entry
form at the Website must be completed to enter and to be eligible to win. Incomplete entries
will be disqualified. Proof of entering information at the Website is not considered proof of
delivery to or receipt by Meijer of such entry.

NOTE: Internet entry must be made by the entrant, only at the Website. Entries made by any other
individual or any entity, and/or originating at any other Website or email address, including but not
limited to commercial sweepstakes subscription notification and/or entering service sites, will be
declared invalid and disqualified for this Sweepstakes. The use of any device to automate the entry
process is prohibited.

b. BY PHONE: For questions and to enter by phone, call the COVID-19 Hotline at
888-535-6136, then press 4 and provide the information requested, to receive one (1) entry
into the Sweepstakes. The COVID-19 Hotline will be available from 8:00 a.m. Eastern
Time (“ET”) to 5:00 p.m. ET Monday-Friday and 10:00 a.m. ET-2:00 p.m. ET on Saturday
and Sunday. All requested information must be provided to enter and to be eligible to win.
Incomplete entries will be disqualified. Proof of providing information is not considered proof
of delivery to or receipt by Meijer of such entry.

Limit one (1) entry per person and per vaccination record regardless of number of COVID-19
vaccination doses received.

WHAT IS THE MICHIGAN COVID HOTLINE AND HOW IS IT INVOLVED WITH THE
SWEEPSTAKES?

The Michigan COVID hotline (866-806-3447) connects Michigan residents with help and answers
from thousands of health and human services agencies and resources right in their communities—
quickly, easily, and confidentially.

Operators have been fielding calls through the Michigan COVID hotline throughout the pandemic and
will continue to assist people who have questions about the vaccine. The hotline is not associated
with the sweepstakes, however, its operators can help with locating vaccine distribution sites and
what people should know before receiving the vaccine.

The hotline has several statewide funding partners, including Michigan Association of United Ways,
which is a key partner in this sweepstakes as well.

HOW WILL WINNERS BE SELECTED AND NOTIFIED?

On or about each of the drawing dates listed above, the prize winner(s) associated with that drawing
will be randomly selected from all eligible entries received in accordance with the deadlines stated
above.

The potential prize winners will be notified by telephone and/or email, using contact information
provided or collected at the time of entry.

The Sweepstakes Entities shall have no liability for any winner notification that is lost, intercepted or
not received by a potential winner for any reason. If, despite reasonable efforts, a potential winner
does not respond within ninety-six (96) hours of the first notification attempt, or if the prize or prize
notification is returned as unclaimed or undeliverable to such potential winner, such potential winner
will forfeit his or her prize and an alternate winner may be selected. Meijer, in its sole discretion, will
attempt to contact up to five (5) potential winners of a prize in accordance with the above procedure,
after which the prize in question may go unawarded if it remains unclaimed. If any potential prize
winner is found to be ineligible, or if he or she has not complied with these Official Rules or declines
the prize for any reason prior to award, such potential prize winner will be disqualified, and an
alternate prize winner may be selected. The odds of winning depend on the number of eligible
entries received in accordance with the deadlines associated with that drawing.

HOW WILL THE MICHIGAN DEPARTMENT OF HEALTH AND HUMAN SERVICES KNOW IF I
HAVE BEEN VACCINATED?

By submitting an entry for the MI Shot To Win Sweepstakes, you agree to allow Meijer to access
your vaccination records to verify that you have received a vaccination. Additional information is
available in the Official Rules.

WHEN WILL THE MI SHOT TO WIN SWEEPSTAKES WINNERS BE ANNOUNCED?

The winners will be announced via virtual press conferences around 10:30 a.m. ET beginning
Monday, July 12, and continuing on consecutive Mondays at 10:30 a.m. Dates and times for
announcements are subject to change.

WHERE WILL THE WINNERS BE ANNOUNCED?

The online press conferences that begin at 10:30 a.m. on Mondays will be livestreamed and will be
available for viewing afterward on the Michigan Department of Health and Human Services Facebook
page at https://www.facebook.com/michiganhhs/.

The winners will also be posted on www.MIShotToWin.com after the announcement.

AM I RESPONSIBLE FOR ANY TAXES ON THESE PRIZES?

For the drawing for Michiganders ages 18 and over, the winner will be responsible for any taxes
associated with this prize. An IRS Form 1099 will be issued to each applicable winner. All federal,
state and local laws and regulations apply.

For the drawing for Michiganders between ages 12 to 17, the funds will be distributed to the
Michigan Education Trust (MET) to manage the scholarship fund for the recipient, who becomes the
beneficiary. More information is available in the Official Rules.

CAN I ENTER THE DRAWING MORE THAN ONE TIME?

Only one (1) entry per person will be accepted and all duplicate entries will be removed prior to the
drawing being held.

IS MY INFORMATION PRIVATE?

Yes. The State of Michigan will not sell any data collected on this website. Acceptance of a prize
constitutes prize winner’s permission for Meijer, the State of Michigan, the Michigan Association of
United Ways and their respective designees to use the prize winner’s name, photograph, likeness,
voice, biographical information, statements and address (city and state) for advertising and/
or publicity purposes worldwide and in all forms of media now known or hereafter developed, in
perpetuity, without further compensation.

ONCE I ENTER, WILL I BE ELIGIBLE FOR ALL THE DRAWINGS?

Yes. Once you enter, your entry will be carried over through all the MI Shot To Win Sweepstakes
drawings for which you are eligible. All winners will be removed from the eligible draw list for
subsequent drawings.

HOW WILL I KNOW IF I HAVE BEEN ENTERED INTO THE DRAWING?

An entry confirmation will be displayed immediately after a successful entry.

WHAT IF I MADE A MISTAKE ON MY ENTRY FORM?

If you make an error, you should re-register with the correct data. We will only include the most
recent entry in the drawing.

I OWE CHILD SUPPORT OR A DEBT TO THE STATE OF MICHIGAN. WILL IT BE TAKEN OUT
OF MY WINNINGS?

CAN I STILL ENTER IF I AM A MICHIGAN RESIDENT BUT WAS VACCINATED IN ANOTHER


STATE?

Vaccination status must be reported to the state of Michigan and will be verified by Meijer against
the Michigan Care Improvement Registry (“MCIR”) database. Work with your health care provider to
ensure immunization data is added to MCIR. If an individual was vaccinated out-of-state, they should
take their immunization record card to their health care provider and the provider would be able to
add it to the MCIR.

Contact the facility where you were vaccinated to request the date your vaccine was administered.

TAKES PLACE?

You must be vaccinated prior to the end of the entry period. For example, for the first daily drawing,
you must be vaccinated on July 1, 2021, and have submitted your entry prior to July 5, 2021, at 11:59
p.m. ET. Thereafter, you must be vaccinated and have your entry submitted by the dates listed on the
schedule above.

IS ANYONE NOT ELIGIBLE?

Certain individuals are not eligible for the drawing. To be eligible, an individual must be a living
Michigan and legal U.S. resident, age 12 or over.

Individuals who are directly involved with the operation of the Sweepstakes, including employees
and independent contractors of Meijer with such direct involvement, are ineligible to enter the
Sweepstakes or win a prize. Employees and independent contractors of the State of Michigan
assisting with the Sweepstakes are ineligible to enter the Sweepstakes or win a prize. Sweepstakes
is void where prohibited by law. Please see the Official Drawing Rules for more information.

WHERE WILL THE MONEY FOR THE PRIZES COME FROM?

The Michigan Department of Health and Human Services is using a portion of its Coronavirus Relief
Funds to fund this innovative public outreach campaign and initiative. The U.S. Treasury, under
the direction of Presidents Trump and Biden, have authorized funds for broad COVID-19 response
purposes. Michigan is using a portion of its unspent funds to share information about the availability
and efficacy of COVID-19 vaccines and to encourage additional Michiganders to become vaccinated
to stop the disease and boost our public health and continued economic recovery.

CAN I SUBSTITUTE PRIZES OR GET CASH ALTERNATIVES INSTEAD OF ACCEPTING A


COLLEGE SCHOLARSHIP?

No cash alternative or prize substitutions will be allowed, except Meijer reserves the right to substitute
prize(s) of comparable value if a prize listed is unavailable for any reason. Prize winners shall be
solely responsible for all federal, state and/or local taxes, and the reporting consequences thereof,
and for any other fees or costs associated with the applicable prize. Any potential prize winner
must sign and have notarized an Affidavit of Eligibility, a Liability Release, and (where imposing
such condition is legal) a Publicity Release (collectively, “Prize Claim Documents”). For the
Scholarship Drawing prizes, the parent/legal guardian of the prize winner must also sign the Prize
Claim Documents on behalf of the winners. If winner fails or refuses to sign and return all Prize Claim
Documents within five (5) days of prize notification (or a shorter time if required by exigencies), winner
may be disqualified and an alternate winner may be selected.

The Sweepstakes Entities make no warranties, and hereby disclaim any and all warranties, express
or implied, concerning any prize furnished by third parties in connection with the Sweepstakes.
WITHOUT LIMITING THE GENERALITY OF THE FOREGOING, SUCH PRIZES ARE PROVIDED
“AS IS” WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, AND THE
SWEEPSTAKES ENTITIES HEREBY DISCLAIM ALL SUCH WARRANTIES, INCLUDING BUT NOT

WHO IS VALIDATING THE EXECUTION OF THE SWEEPSTAKES PROCESS?

Ernst & Young LLP will be assessing the execution of the Sweepstakes process, including whether
the winner selection and winner validation process is in accordance with Meijer’s processes.

Acceptance of a prize constitutes prize winner’s permission for Meijer, the State of Michigan, the
Michigan Association of United Ways and their respective designees to use prize winner’s name,
photograph, likeness, voice, biographical information, statements and address (city and state) for
advertising and/or publicity purposes worldwide and in all forms of media now known or hereafter
developed, in perpetuity, without further compensation.

Entrants agree that the Sweepstakes Entities (A) shall not be responsible or liable for, and are
hereby released from, any and all costs, injuries, losses or damages of any kind, including, without
limitation, death and bodily injury, due in whole or in part, directly or indirectly, to participation in the
Sweepstakes or any Sweepstakes-related activity, or from entrants’ acceptance, receipt, possession
and/or use or misuse of any prize, and (B) have not made any warranty, representation or guarantee
express or implied, in fact or in law, with respect to any prize, including, without limitation, to such
prize’s quality or fitness for a particular purpose. The Sweepstakes Entities assume no responsibility
for any damage to an entrant’s or any other person’s computer system or wireless phone which

in its sole discretion to disqualify any individual who is found to be tampering with the entry process
or the operation of the Sweepstakes or the Website, to be acting in violation of these Official Rules,
or to be acting in an unsportsmanlike or disruptive manner, or with the intent to disrupt or undermine
the legitimate operation of the Sweepstakes, or to annoy, abuse, threaten or harass any other person,
and Meijer reserves the right to seek damages and other remedies from any such person to the
fullest extent permitted by law. No mechanically reproduced, illegible, incomplete, forged, software-
generated or other automated multiple entries will be accepted.

Meijer reserves the right to modify, extend, suspend, or terminate the Sweepstakes if it determines, in
its sole discretion, that the Sweepstakes is technically impaired or corrupted or that fraud or technical
problems, failures or malfunctions or other causes beyond Meijer’s control have destroyed or severely
undermined or to any degree impaired the integrity, administration, security, proper play and/or
feasibility of the Sweepstakes as contemplated herein. In the event Meijer is prevented from awarding
prizes or continuing with the Sweepstakes as contemplated herein by any event beyond its control,
Meijer shall have the right to modify, suspend, or terminate the Sweepstakes. If the Sweepstakes
is terminated before the designated end date, Meijer will (if possible) select winners in a random
drawing from all eligible, non-suspect entries received as of the date of the event giving rise to the
termination. Inclusion in such drawing shall be each entrant’s sole and exclusive remedy under such
circumstances.

These Official Rules cannot be modified or amended in any way except in a written document issued
by a duly authorized representative of Meijer. The invalidity or unenforceability of any provision of
these Official Rules shall not affect the validity or enforceability of any other provision. In the event
that any provision is determined to be invalid or otherwise unenforceable or illegal, these Official
Rules shall otherwise remain in effect and shall be construed in accordance with their terms as if the
invalid or illegal provision were not contained herein.
