SERVICE AGREEMENT

KNOWN TO ALL MEN THESE PRESENTS:

The City of Mabalacat, a local government unit established and existing

under the laws and regulations of the Republic of the Philippines, with office
address at Rizal St. Barangay Dolores, Mabalacat City, Pampanga, Philippines
LAYO, MPA represented by Ms. HAPPY S. PELAYO, Vice President for Finance &
r Finance & Administration, hereinafter referred to as FIRST PARTY;
-and-

JENNY G. GATPOLINTAN of legal age, Filipino and with postal address at

Tastes from the Greens – Bamban, Tarlac hereinafter referred to as SECOND
PARTY.

WITNESSETH:

WHEREAS, FIRST PARTY entered into an agreement with Taste from the
Greens;
WHEREAS, the purpose of this Agreement is to fulfill the obligation of the
SECOND PARTY to provide drinks to the Full-time Instructor and Non-
Teaching Personnel with teaching loads of the FIRST PARTY and;
WHEREAS, for FIRST PARTY to compensate the service and product provided
by the SECOND PARTY;
WHEREAS, all parties are cognizant of the need for cooperative endeavor and
fully aware of the mutual advantages and benefits that could be experienced in
TPOLINTAN undertaking this agreement;

NOW, THEREFORE, for and in consideration of the foregoing premises, the
Parties agree as follows:

I. ROLES AND RESPONSIBILITIES

OBLIGATIONS OF THE FIRST PARTY:

A. Office of the Vice President for Finance and Administration as the FIRST
Party shall pick-up the drinks from Taste from the Greens store Bamban
branch;

B. Throughout the event, the FIRST PARTY will assist the SECOND PARTY
with the delivery of drinks as a gift to event participants and additional
assistance as needed.

C. The FIRST PARTY shall pay a fifty (50) percent down payment prior to
the event and the remaining seventy (50) percent payment after the
event.

 OBLIGATIONS OF THE SECOND PARTY
A. The SECOND PARTY offers to supply eighty (80) individuals with ordered
beverages with free customized stickers for pick-up during the event on
AYO, MPA October 14, 2022, as well as one (1) free drink for every ten (10)
Finance & purchased drinks, plus one (1) additional free for being an associated
business for the event;

B. If the aforementioned participants fail to attend the event, the SECOND

PARTY agrees to provide coupons to be redeemed at the event and at the
store; the coupon will expire at the end of the year 2022.

II. AMENDMENT

No amendment or variation of the terms of this Agreement shall be effective

unless made in writing and consented to by both parties hereto.

III. FINAL PROVISION

Failure on the part of either party concerned to follow or abide by the

provisions stipulated herein shall be deemed sufficient ground to revoke this
Service Agreement.
