2. Identification of new hazardous factors before project commencement (Note:
Mainly including but not limited to the following aspects)
2.1 hazardous factors caused by personnel, technology and equipment / facility
changing
a) If there is new person or key post adjusted person who may be not very familiar

with equipment operation or lack of site experience in the well: (Yes √ , No □)

b) Whether the well uses any new process and new technology: (Yes □, No √ )

c) Equipment and facility changed for the well may have intrinsic fault, safety
protection defect, unimproved technical specifications, or without technical

specifications: (Yes □, No √ )

d) Whether the well uses any new materials that may do harm to people or pollute

environment: (Yes □, No √ )

e) If there is any change in well design: (Yes □, No √ )

f) Green hand easy injury by lack of experience.

g) Key position changed frequently, and new person lack of operation skills and
communications.
h) Unknow the lithology of the formation due to the first time drilling at this area,
May caused stuck and kick. And the damage risk on the drill pipe.
2.2 Main risk hints in geology and engineering design

a) If there is any shallow gas zone in the well (Yes □, No √ )

2
b) If there is toxic and hazardous gas such as H2S、CO etc. in the well:

(Yes √ , No □)

c) If the well is rich in high gas pressure and high pressure of the well and plentiful
Sulfur as specified in the detailed rules for implementation of well control:

(Yes □, No √ )

d) More wiper trip needed, due to the new lithology unknow.

e) Keep good properties of the mud, Make sure enough lubrication on the mud in
the downhole.
2.3 hazardous factors caused by geological and environment
a) If there is any water (steam) injection well and oil (gas) production well

surrounding the well: (Yes √ , No □)

b) If there are three aboveground wires (high and low voltage wires, communication

wires) above the well site ground: (Yes √ , No □)

c) If there are three underground lines (oil, gas, water pipelines, and underground

cable) under the well site ground: (Yes √ ,, No □)

d) If there is any residential building, school, factory and mine (including mining
unit which produce underground resources) and national defense facility:

(Yes □, No √ )

e) If there is any environmental sensitive area such as riverbed, marine beach, lake,
salt pan, reservoir, aquaculture, water resources, wild animal and plant protection

area surrounding the well: (Yes □, No √ )

f) If there is production facility of relevant parties on the well site, such as old
wellhead, oil tank, heating furnace (open fire), transformer, oilfield constructed

joint station, gathering, transportation station, etc. : (Yes √ , No □)

g) If there is road across the well site, and if there is any express highway and

railway in 500m around the well site: (Yes □, No √)

h) Whether the well site is a new location, and whether there is any risk of well site
collapse or landslide, foundation subsiding and mud pit (waste water reservoir)

3
collapse: (Yes □, No √ )

i) If the well site is located on hill top, there is any risk of landslip and debris flow:

(Yes □, No √ )

j) If there is any endemic disease or infectious disease in the area where well site is

located: (Yes √ , No□)

k) If there is local custom taboo and religious belief in the area where well site is

located: (Yes √ , No □)

l) If the public security environment is good surrounding the well: (Yes □, No √ )

m) If there is any underground concealed substance, cultural relics and historic site

surrounding the well: (Yes □, No √ )

n) If there is blind area of communication surrounding the well: (Yes □, No √ )

o) If there is storm, lightning strike, flood, cyclone, storm surge and sand storm etc.

in the well: (Yes □, No √ )

p) If there is high temperature heat stroke, and rotten and deteriorated food such as

vegetables in the locality: (Yes √ , No □)

q) If there is operation in cold winter: (Yes √ , No □)

r) If there is operation in rainy season: (Yes √ , No □)

s) Other risks : Strong season wind caused the cable down to the mast or twist on
the mast from the TDS..
t) Production wells have been working in the same plot, The risk of fire and
explosion probability would be higher.
u) Pandemic COVID-19 is spreading fast in Iraq. It may infect personnel and cause
operation suspended.
v) Local landlord block the road and security issue should pay highly attention to.
3. Risk elimination, reduction and control measures established for new
hazardous factors Identified before operation, One by One. (Note: In term of

4
common operation hazards elimination or reduction, referring to Drilling /
Workover Operation HSE Instructor)
A\ Risk from personal changed: HSE orientation & training according to training
matrix as per HSE-MS, Safety Training Observation Program, etc.
B\ Risk from new process & technology : the same above and management program
of technology change, etc.
C\ Risk from equipment change and repair: the same above, lock out and tagout
program management program of equipment change, work permit, etc.
D\ Green hands. To implement Green Hands Policy including buddy system and
person to person training at rig site. Bule helmets issued for green hands. No
supervision, no work.
E\ risk identifying of others: Stop Work Policy,
Hazard hunt program including STOP card,
Pre-job safety meeting,
JSA,
Permit to Work System
Other HSE tools and risk control measures, etc
F\ COVID-19 prevention. All personnel shall be trained according to “CNPC
Covid-19 management instructions”, “GWDC Covid-19 management instructions”,
“GWDC SOP Against COVID19” to manage Covid-19. All employees shall follow
Eriell covid-19 sop.
Note: 13 wells are designed on wellpad K. Fly camp and the drilling crew will
remain the same. The main risks remain the same for all 13 wells.
