2:00PM received patient with D5W IL at 200cc level infusing well at 20 gtts/minute
V/S Taken and recorded as follows: T= 37.5C; PR 61/min; RR 18/minute
Still with productive cough noted
All due meds given as ordered
6:00PM above IVF consumed followed with D5NM 1L regulated at 20 gtts/min as ordered
Patient drink 1 glass of juice
7:00 patient vomited 3x amounting to 1 glass/vomitus
Complained of severe abdominal pain
7:15 BM watery 6x amounting to 1OZ /BM
7:30 Vomitted 3x amounting to 2OZ/vomitus
8:00PM referred to Attending Physician with the following orders:
-give buscopan 2cc IVT stat
-D/C present IVF temporarily; instead, give D5LR 1/4L to run for 2 hours;then
resume present IVF
-for stool exam stat
Give paracetamol 1cc IVT for fever every 4 hours PRN
Insert catheter and connect to urine bag
8:30 Buscopan2cc given as per doctors order
afebrile
Present IVF D/C temporarily as ordered at the level of 830cc level; D5LR 1/4 L inserted
regulated to run for 2 hours
9:00PM catheter inserted connected to urine bag
10:00PM D5LR1/4L consumed and terminated ; D5NM 1L resumed as ordered,regulated at
20gtts/min.
10:00PM endorsed with D5NM 1L at the level of 700cc infusing well; with catheter intact
connected to urine bag with urine yellowish in color at 300cc.

Paracetamol 1 tbsp every 4 hours PRN for fever

D5NM = 170
D5NM = 130 (170+130= 300)
Chloramphenicol 2cc = 2ml
Total = 994ml
OUTPUT
1 glass vomit = 720ml
1oz bm = 180ml
2oz vomit = 120ml
300cc urine = 300ml
Total =1320ml
