AFFIDAVIT

I, ARHSOLE C. JALLORINA, of legal age, Filipino, married and a

resident of Cabugao Sur, Pavia, Iloilo, Philippines, after having been duly sworn to
in accordance with law, hereby depose and state:

1.) That I am a member of the 33MICO, 3MIB, 3ID PA of the Armed

Forces of the Philippines with a rank of Staff Sergeant assigned at Camp
Gerona, Brgy. Minoyan Murcia, Negros Occidental, Philippines;

2.) That I was late in the turnover of my issued firearm which the Armed
Forces of the Philippines issued to me specifically a 9MM Glock 17
Pistol with a serial number AFP041253;

3.) That this is because I was deployed in Negros Island which is far from
Iloilo City;

4.) That I execute this Affidavit as a requirement from the Armed Forces of
the Philippines to turnover my issued firearm at the 6 th Forward Service
Support Unit, Camp Gen. Martin Delgado, Iloilo City and for whatever
legal purpose it may serve.
