Location : Tank 1 Vertical tendon V-236
Issuer

During stressing of vertical tendon V-236 Tank 1, SPAN was process stressing work with out
SPCC QC join witness inspection.

Therefore, SPAN is strongly requested to take necessary actions below and mitigation follow ITP
1) To investigate the root causes of this problem
2) To perform any inspection mush had SPCC QC to join inspection hold point all time
(see attachment)
