months.

-Patient states that he is fine in the past few days.

-Patient did not develop any health conditions or illnesses as an adult that would require him to
have medical treatment or surgery.
C. Family health
Patient’s family did not live near hazardous sites or any major disasters that may have affected
their health. Patient’s family usually eat bacon eggs and pancake for breakfast. Patient is allergic
to eating mangoes. Patient’s mother does not have any difficulty when she is carrying the patient
in her womb. Patient’s family has a history of heart attack. Patient does not have any health
concerns in his family.

Patient does exercise everyday and lives in a peaceful environment.

Patient plays basketball and does exercise daily.

Patient usually eats breakfast at 7am, lunch at 12 pm, and dinner at 7 pm. Patient urinates 7 times a day
and defecate thrice a week. Patient does exercise everyday. Patient does not have any trouble growing up.
Patient usually sleeps at 11 pm or 12 midnight depending on his situation, if he is not busy doing school
works he sleeps at 10 pm. Patient states that he feels good. Patient states that he is satisfied with his work.
Patient does not have any difficulties in having sexual relationships. Patient stated that he sometimes
drink beer and always do exercise. Patient’s religion is a Christian which gives him a positive effect on
his spiritual life.
