Scenario:
Patient Y, 39 years old, was admitted in
Medical Ward with a diagnosis of
Meningitis. Upon, assessment, you noticed
that his temperature was 40 degrees, was
teary eyed, with flushed skin that was warm
to touch, with dry lips and mucous
membrane. He said, “I feel so hot all over.”

1. Identify the objective data based on the scenario.

• 4-degrees temperature
• Teary eyed
• Flushed warm skin
• Dry lips
• Dry mucous membrane

2. What assessment technique/s will be utilized to assess the patient? Why? (Explain in 3-
5 sentences)
The technique that will be used here is Inspection and palpation. Since the patient is
diagnosed with Meningitis we need to inspect, skin color, temperature and body
movement. We need to inspect for positive Brudzinski’s sign and Kernig’s sign. In
palpation we need to palpate if there are any unusual markings, signs or lumps in the skin
of the patient.

3. Explain the importance of performing the appropriate assessment techniques in
identifying patient’s health problems. (Explain in 3-5 sentences)
By using the appropriate assessment techniques, we can determine more information
about the condition of the patient. And by gathering information about the client by using
these techniques, we can now plan an objective and an intervention in making a nursing
care plan for this patient. Furthermore this will make us learn more about the disease that
caused the condition of the patient to help us improve more our care in the future.
