My wife, Smt. Kodakanti Rajani,45 years(my family dependent) suffering with chronic lungs breathing
problems( INTERSTITIALlung disease) since 2016 and recently admitted in SVIMS hospital in ICU under
emergency AS IN-PATIENT on 01-07-2022 and discharged on 13-07-2022 AND paid paid the amount of medical
bill by me .The necessary medical re-imbersement bill along with certificates is here with submitted for sanction
as early as possible please.
