INFORMED CONSENT

Please read this document thoroughly before signing. The effect of

this document is to release Our Lady of Fatima University – College of
Criminal Justice from any liability resulting from your participation
in “OATH TAKING CEREMONY” of Criminal Justice Students Society and
LEADERSHIP SEMINAR.

I Nicole Venus Montante (First Name, Middle Name, Last Name),

of legal age 18 , born on February 18, 2004 (Month, Day, and Year),
with Student ID Number 06220000502, under the School of Criminology
(Academic Program) and currently residing at BLK 3 LOT 6, RIVIERA
HEIGHTS, BARANGAY LABAS, SANTA ROSA, LAGUNA (complete permanent
address), son/daughter of Enrique P. Nabora, is willing and will
voluntarily participate in the “OATH TAKING CEREMONY” of Criminal
Justice Students Society and LEADERSHIP SEMINAR to be held at
Valenzuela Campus and accepting the potential risks posed by the said
activity, including the health risk associated with the COVID-19
pandemic.

My parents or legal guardian and I understand that my

participation in the school activity is voluntary. However, it may
include activities that expose me to certain risks, including but not
limited to all risks associated with the COVID-19 virus, such as
serious illness, hospitalization, or death, as well as risk for
individuals with commodities such as diabetes, lung disease, and
heart disease.

I understand that the College of Criminal Justice may

immediately remove me from school activity if I become ill or
symptomatic. Considering being permitted to participate in the school
activity, I now release, waive and discharge the College of Criminal
Justice from any actions, damages, claims, or demands.

My parents or legal guardians are fully aware that I have to

submit the health declaration. Furthermore, I am aware that I must
always adhere to the established health and safety measures, physical
distancing, proper hand-washing, and wearing face masks.

My parent or legal guardian understands that Our Lady of Fatima

University – College of Criminal Justice does not require me to
participate in the school activity. However, despite the possible
dangers and risks, I want to do so.
Therefore, I agree to assume all of the risks and responsibilities
associated with the school activity.
