A RESOLUTION AUTHORIZING THE BARANGAY HEALTH CARE WORKER VILMA D. LAMBIT

TO INCASH CHECK NO. 999227 WITH A CORRESPONDING AMOUNT OF PHP 880.00 PESOS
ONLY AS REIMBURSEMENT OF HIS/HER TRAVELLING EXPENSES IN HIS/HER VAROIUS
OFFICIAL BUSINESSESS PERFORMING HIS/HER DUTIES AS BARANGAY HEALTH CARE
WORKER OF THE BARANGAY.

WHEREAS, the Sangguniang Barangay of Sampao allocated funds for travelling expenses for barangay
officials and employees in performing their duties and responsibilities;

WHEREAS, that there is an allocated fund from MOOE CY 2021 and was already deposited at the
official bank depository of the barangay, the Land Bank of the Philippines, Naval, Biliran Branch;

NOW THEREFORE, on motion of Hon. Alberto T. Asumbrado duly seconded by the rest of the
Sangguniang Barangay present, be it;

RESOLVED AS IT HEREBY RESOLVED, to authorize the Barangay Health Care Worker Vilma D.
Lambit to incash check no.999227 with a corresponding amount stated above and its purpose;

RESOLVED FURTHER, that copies of this resolution be furnish to the offices concern for their
information and guidance;
