Customer: PETRONAS CHEMICALS FERTILISER SABAH SDN BHD 1. Velocity vibration at all measurement points seems within acceptable
Plant: SAMUR limit, however there are some elevated of bearing fault frequencies.
Event Date: 3/2/2021 2. Acceleration envelope (g’s) reading that use to evaluate bearing
Event Type: Ad-Hoc Vibration Measurement & Analysis condition shows trending up level at the motor bearings with Fault
Equipment ID: K-39-91 P2 level.
Report Date: 5/2/2021 3. The envelope reading predominantly by non-synchronous peaks which
are the bearing fault frequencies with amplitude modulation waveform
shows the bearing condition should getting attention.

Event Description Recommendation

Based on monthly CBM routine, the K-39-01 P2 unit has been put into
watchlist due to trending of acceleration envelope data and abnormal
spectrum pattern suspected bearing defect.
On 3rd February 2021, a confirmation survey was attended with the intention
to confirm the machines that has been put into our watchlist and attention.
The K-39-01 P2 bearings has been confirmed are within defect stage.
- Equipment may continue to operate however bearing replacement
should be plan in the next opportunity.
- Grease the motor bearing in the correct amount necessarily to prolong
the bearing until next bearing replacement.
