CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

That this CONTRACT OF LEASE is made and executed by and between:

That ooooooooooooooo, of legal age, married, Filipino citizen and a resident of Mabini St.,
Catbangen, San Fernando City, La Union, hereinafter referred to as the LESSOR, and

AAAAAAAAAAAAAAAAAA, also of legal age, married, Filipino citizen and a resident of

Florentino St., Catbangen, San Fernando City, La Union, hereinafter referred to as the LESSEE,

W I T N E S S E T H:

That the LESSOR is the owner of a building located at Pennsylvania Ave., Catbangen, San
Fernando City, La Union and lessor desires to lease said building to herein lessee and LESSEE accepts to
lease, as VIDEOKE BAR, under the following terms and conditions, to wit:

1. Period of lease shall be one (1) year, beginning __________________________ and subject
for renewal upon the option of both parties;

2. Monthly rental shall be SEVEN THOUSAND PESOS (P7,000.00) Philippine Currency, payable
every 15th day of the month;

3. Lessee has given P7,000.00 to lessor as deposit thereto;

4. Payment for light and water shall be the sole account of the lessee;

5. Failure to pay monthly rental of the lessee, lessor shall give five (5) days extension to lessee
to pay, after five (5) days of non payment, ten percent (10%) increase of the monthly rental
of the lessee;

6. Lessee shall abide with all the terms and conditions of this contract;

IN WITNESS WHEREOF, both parties have hereunto affixed their signature this _____ day of
