CLEANING AND DECORATING OF CLASSROOMS FOR THE GRAND OPEN HOUSE (GOH) CONSENT FORM

To whom it may concern,

I __________________________, as the parent/guardian is giving consent for my child,

__________________________, of 9 – Feynman to volunteer in the cleaning and decorating of
classrooms which will be held on April 28, 2022, 2-4pm onwards at Muntinlupa Science High School for
the incoming Grand Open House that will occur on Saturday, April 30, 2022.

Knowing the diligence applied by the teachers and other students and leaders to sustain the safety of
my child during this activity, we shall not hold the school, its teachers, officers, or any of the school staff
and leaders thereof, responsible for any unpredicted mishaps or accidents beyond their control.
