AFFIDAVIT OF LOSS

I, CYRIL C. FRANCISCO, Filipino, of legal age, married, and a resident of Brgy.

Lupit, Batan, Aklan, after having been duly sworn to in accordance with law hereby
depose and say:

1. That I am a holder of Professional Driver License issued by the Land

Transportation Office (LTO) bearing numbers F07-07-000672 which
expired on January 13, 2017;

2. That sometime on December 23, 2016, I lost my wallet where I kept the
said license and my other identification cards;

3. That despite diligent efforts to locate it, the same could no longer be
found and is deemed lost; and

4. That I am executing this affidavit to attest to the truth and veracity of the
foregoing statement;

JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE: ONE AND THE SAME PERSON

WE, CYRIL C. FRANCISCO and ELYN P. URETA, both of legal ages,

Filipinos, married, respectfully and both are residents of Brgy. Lupit, Batan, Aklan, after
having been sworn in accordance with the law hereby depose and state that;

1. We personally know Maria Lusy Macahilig a.k.a. Luz Macahilig

Tulio a.k.a. Ma Luz Macahilig Tulio; who is our longtime neighbor and
acquaintance at Lupit, Batan, Aklan, and the fact of her birth is registered at the
Local Civil Registrar of Batan, Capiz (now Aklan);

2. We know for a fact that the person by the name of “Maria Lusy
Macahilig” as appearing in her Certificate of Live Birth, the name “Luz Macahilig
Tulio” as appearing in her Social Security System (SSS) records, and the name
“Ma Luz Macahilig Tulio” as appearing in her PhilHealth records, refer to one
and the same person and that her true and correct name is Maria Lusy
Macahilig;

3. We are not in any way related to any of the foregoing person within the
fourth civil degree either by consanguinity or affinity, nor do we have any
pecuniary interest in this matter whatsoever;

4. To remove confusion as to the three (3) names, we hereby declare

that the names Maria Lusy Macahilig, Luz Macahilig Tulio, and Ma Luz
Macahilig Tulio refer to one and the same person whose registered name is
Maria Lusy Macahilig;

5. We are executing this affidavit to attest to the truthfulness of the

foregoing statements and to establish the real identity of Maria Lusy Macahilig
and for all legal intents and purposes it may serve.
