JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE: ONE AND THE SAME PERSON

WE, CYRIL C. FRANCISCO and NENET S. TUOS, both of legal ages, Filipinos,
married, and are residents of Brgy. Lupit, Batan, Aklan, after having been sworn in
accordance with the law hereby depose and state that;

1. We personally know REYNAND MATUTINA a.k.a. REYNAND

BERMUDO MATUTINA; who is our longtime neighbor and acquaintance at Lupit,
Batan, Aklan, and the fact of his birth is registered at the Local Civil Registrar of
with First Name REYNAND, no Middle Name and Last Name MATUTINA;

2. We know for a fact that the person by the name of “Reynand Matutina” as
appearing in his Certificate of Live Birth, the name “Reynand Bermudo Matutina” as
appearing in her Social Security System (SSS) records, PhilHealth records, and
Commission on Election Records refer to one and the same person and that his true
and correct name is Reynand Matutina only;

3. We are not in any way related the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest in
this matter whatsoever;

4. To remove confusion and to correct the name appearing in his official

records, we hereby declare that the names Reynand Matutina, and Reynand
Bermudo Matutina refer to one and the same person whose registered name is
REYNAND MATUTINA;

5. We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish the real identity of Reynand Matutina and for all legal
intents and purposes it may serve.

That on account of the said accident, I suffered physical injuries and was brought
and confined at Dr. Rafael S. Tumbukon Memorial Hospital , Kalibo, Aklan on July 9,
2016 until July 15, 2016 for medical treatment incurring estimated expenses of
P1,470.60; and

That I am executing this affidavit to attest to the foregoing facts for all legal
purposes.
